In recent years,with the social progress and economic development,standards of China's health services and the per capita life expectancy have been increased,while the number of patients with acute infectious diseases has decreased.On the other hand,as the improvement of living conditions and changes in people's lifestyles,the number of patients with chronic non-communicable diseases has increased significantly.Chronic diseases,such as cardiovascular,cerebrovascular,and malignant tumors,have become the main cause of people's mortality.The number of deaths caused by chronic diseases has accounted for 86.6%of the total deaths in the country,contributing to nearly 70%of the total disease burden.Harm has also been caused to people's lives because of chronic diseases,which is hardly ignored.It not only has a huge impact on society,families,and individuals,but also imposes a heavy financial burden on them,reducing the people's quality of lives.In this context,studying the vulnerability of health and poverty and its related factors among patients with chronic diseases is the key to improve health-related policies and reduce the likelihood of people falling into poverty.Previous studies on poverty vulnerability have mainly focused on economic and ecological change.Although some scholars have pointed out that health has a very important impact on poverty vulnerability,few scholars have conducted research on its in-depth causes.Therefore,this paper,from the perspective of health and poverty vulnerability of chronically ill patients,studies theoretical and practical significance due to the reduction of health and poverty vulnerability.This study is based on the theory of poverty cause of health,disease risk theory,and healthy human capital theory,and draws on relevant research on poverty,poverty vulnerability,and health and poverty vulnerability among chronically ill patients.It defined the connotation of the vulnerability of health and poverty,conducted two rounds of expert consultation by using the method of Delphi expert consultation,and constructed the Evaluation Index System for Health and Poverty Vulnerability among Chronic Patients.In order to verify the applicability of the established index system among chronically ill patients,this study investigated the northern area of Jiangsu province where economic development was relatively weak.The chronical patients whose annual income was under 3,000 yuan and 6,000 yuan respectively according to China's poverty standard in 2016 and Jiangsu's poverty alleviation standard were selected from three cities,called Huai'an,Lianyungang and Yancheng respectively.The results show that under the 3,000-yuan poverty line,the average poverty vulnerability of three cities of northern Jiangsu is 0.1663,and the vulnerability rate is 0.0755.Under the 6,000-yuan poverty line,the average vulnerability is 0.2941,and the vulnerability rate is 0.2480.Descriptive statistics and one-way ANOVA were used to investigate the current status of health and poverty vulnerability among chronic patients 'with different characteristics.It found that males were more vulnerable than females;younger patients are more vulnerable to poverty and vulnerability;rural vulnerability is higher than urban;the longer the education period,the less vulnerable to poverty;compared with other occupational types,farmers are more vulnerable;patients with worse self-assessment of health is more vulnerable;patients with malignant tumors are the most vulnerable group;patients with a disease duration of less than 5 years are the most vulnerable group;widowhood is the most vulnerable.Patients with better self-evaluation of health is less vulnerable;the higher the BMI index,the less vulnerable to poverty.Through the quantile regression analysis of the factors affecting the health and poverty vulnerability of patients with chronic diseases,the study found that poverty line of under 3000 and 6000 yuan had a negative impact on the health and poverty vulnerability of patients with chronic diseases at higher quintiles,and the significant factors include:gender,age,years of schooling,self-rated health,chronic disease types,chronic disease duration,BMI index,and satisfaction of medical insurance reimbursement.In conclusion,this study suggested that policy support for key populations should be strengthened,the medical and health service system should be improved,the medical welfare system should be perfected,and the healthy status should be increased,in order to reduce the health and poverty vulnerability of chronical patients.
