Objectives:The study aimed to explore the incidence rare and distribution characters of cerebral microbleeds(CMBs)and to analyze the correlation with common risk factors of vascular diseases including age,gender,hypertension,hypercholesterolemia,diabetes,etc.,and to analyze the relationship between the number and location of CMBs and MoCA score,in order to find the related risk factors and the impact of CMBs on cognitive impairment.Methods:1?125 people were divided into three groups: Alzheimer's disease group(AD group),amnestic mild cognitive impairment group(aMCI group),normal control group(NC group).The MR imaging of all people was dectected by a radiologist and 2 doctors who didn't know medical history.They record and assess the number and distribution of microbleeds.2 ? The incidence rate of CMBs in the three groups was calculated,and the clinical data of the three groups were compared using statistical methods,including gender,age,hypertension,diabetes,hypercholesterolemia,coronary heart disease,family history,smoking history,drinking history,MMSE score,MoCA score,and the number of CMBs,to analyze whether there were differences between the 3 groups.3?The number of cerebral microbleeds in whole brain and in each brain region were calculated and compared by statistical methods to analyze whether there were differences between the 3 groups.4 ? Correlation analysis of CMBs quantity: to explore the correlation between age,gender,hypertension,diabetes,hypercholesterolemia,smoking history,drinking history and CMBs quantity.5?Risk factor analysis of MoCA score: MoCA score was used as the dependent variable,gender,age,hypertension,hypercholesterolemia,diabetes,smoking history,drinking history,and the number of CMBs were used as the independent variables for multiple linear regression analysis to explore the risk factors affecting MoCA score.6?The scores of visual space structure and executive function,naming,attention,read-beat,calculation,language,abstraction,recall,and orientation of patients with CMBs in different parts were counted,and the effects of CMBs in different parts on a certain aspect of cognitive function were analyzed.Results:1?Among the three groups of patients,69 patients were in the AD group,and 28 patients were CMBs positive(40.6%).Among the 29 patients in the aMCI group,6(20.7%)were CMBs positive.There were 27 cases in NC group,among which 6 cases were CMBs positive(22.2%).Gender,age,MMSE score,MoCA score and CMBs quantity were statistically significant among the three groups.2 ? The region of CMBs in AD group shows a lobar predominance.In AD group,the number of CMBs in lobar is as large as the number of CMBs in infratentorial and deep.In aMCI group,the number of CMBs in lobar is as large as the number of CMBs in infratentorial and deep.In normal control group,the number of CMBs in infratentorial and deep is two times as large as the number of CMBs in lobar.For the microbleeds counts,significant differences were found between AD group and aMCI group in the parietal lobe and whole brain,and between AD group and NC group in the parietal lobe and the temporal lobe and the whole brain,and among 3group in he parietal lobe and the temporal lobe and whole brain.3?The number of CMBs was positively correlated with age.The older patients were,the more CMBs they had.4?Gender,age,and the number of CMBs have significant influence on MoCA score,and are independent influencing factors of MoCA score.Among them,gender has a significant positive impact on the dependent variable,and the MoCA score of male subjects is lower than that of female subjects.Age had a significant negative effect on MoCA score.The older the age was,the lower the MoCA score was.The number of CMBs had a significant negative impact on MoCA score.The more CMBs were,the lower the MoCA score was.5 ?MoCA score,visual space structure and executive function,calculation,and delayed recall scores were significantly different in the patients with CMBs positivity and CMBs negativeness in the subcurtain group.MoCA score,visual space structure and executive function,attention,calculation,language and orientation were significantly different in the deep group of CMBs positive and CMBs negative patients.MoCA score,visual space structure and executive function,attention,read-beat,calculation,language,abstraction and orientation were significantly different in patients with CMBs positive and CMBs negative in the lobes group.Conclusion:1 ? The number of CMBs was significantly correlated with cognitive impairment.The more CMBs are,the lower the MoCA score is,and the more severe the cognitive dysfunction is.2?Different parts of CMBs have different areas of cognitive impairment.3?Compared with the NC group,AD patients have more CMBs manifestations in the parietal lobe and temporal lobe,and the quantization of cerebral CMBs is expected to be a candidate indicator for the diagnosis of alzheimer's disease.4?The number of CMBs was positively correlated with age.The older patients were,the more CMBs they had.
