Backgrounds:With the development of the cerebrovascular disease research and the imaging technology,Cerebral small vessel disease gradually gives rise to the attention of the people,and the cerebral microbleeds have become a research hotspot.Cerebral small vessel disease causes brain a few bleeding or red blood cells leaking out hemosiderin deposition,the application of high intensity magnetic resonance imaging(MRI)or gradient echo T2 weighted magnetic resonance imaging(MRI)and magnetic sensitive weighted imaging(SWI),can be found that the pinhole sample low signal.The related risk factors have not yet reached an consensus,More researches is needed.Objectives:Based on the reasearch of related risk factors of cerebral microbleeds to advice patients positive prevention for high risk factors.Through the analysis of cognitive function in patients with cerebral microbleeds to detect cognitive function change earlier in cerebral small vessel disease patients,guide to improve the cognition of drug therapy and the function of the specific training.At the same time guide combined cerebral microbleeds patients thrombolysis,antithrombotic,anticoagulation of risk assessment.Methods:We collected into the index of patients age,gender,history of hypertension,diabetes,fibrinogen,low density lipoprotein cholesterol,total cholesterol,triglyceride,hypersensitive c-reactive protein and homocysteine,smoking,intemperance,history of aspirin use,history of cerebral infarction,cerebral hemorrhage,cerebral white matter loose,lacunar cerebral infarction and related clinical treatment.All the patients ofentering the group need SWI to determine 136 patients divided into CMBs positive group and CMBs negative groupe,bleeding including tentorium group(cerebellum,brainstem),deep group(the thalamus,basal ganglia)and lobes(frontal,top,pillow,temporal lobe).According to the cerebral microbleeds location,we found that patients with visual space and executive function,naming,attention,language,abstract,delayed recall,directional force scored on various aspects is different between the each groups.Results:The 136 patients,cerebral microbleeds positive in 61 cases,75 cases of negative.The clinical data of two groups of patients with single factor analysis,high blood pressure,high blood pressure,number of years,systolic blood pressure on admission,diastolic blood pressure on admission,history of cerebral infarction,cerebral hemorrhage medical history,platelet aggregation in treatment of osteoporosis,cerebral white matter,MMSE,ADL,Mo CA,visual space and directional force,naming,attention,executive function are significantly.Hhypertension is a independent risk factors of the CMBs.According to the number of CMB classification is divided into level 1,level 2,level 3,a total of three groups CMB2 grade level and CMB1 group compared with the years age,high blood pressure,serum homocysteine,MMSE,Mo CA exist significant differences.Levels CMB3 group and CMB1 group compared with the years age,hypertension,MMSE,Mo CA,ADL exist significant differences.The use of antiplatelet aggregation is the independent risk factor of tentorium CMBs patients.Hypertension is a independent risk factor for deep brain and lobe CMBs.Lobes group and deep group with CMBs patients Moca scores,visual space and directional score,naming,attention,executive function was statistically significant.Tentorium group with Moca scores,visual space and micro hemorrhage patients,naming,attention,executive function delayed recall,directional score was statistically significant.Conclusion:(1)The risk factors associated with cereal microbleeds :(1)Hypertension is independent risk factor for the lobes and deep CMBs;,hypertention,hypertention years,systolic blood pressure,diastolic blood pressure,cerebral hemorrhage medical history;(2)the use of antiplatelet aggregation is the independent risk factor of tentorium CMBs patients.(2)CMBs and cognitive dysfunction:(1)Compared cerebral microbleeds positive with CMBs negative,the cognitive function differetly;(2)Lobes group and deep group with CMBs patients visual space and directional score,naming,attention,executive function was statistically significant.Tentorium group with visual space and micro hemorrhage patients,naming,attention,executive function delayed recall,directional score was statistically significant.(3)The more number of cerebral microbleeds,the poorer cognitive function and daily life ability.
