Objectives To evaluate the value of preoperative neutrophil to lymphocyte ratio(NLR)in evaluating the prognosis of patients with duodenal papillary carcinoma.Method The clinical data of 83 patients with duodenal papillary carcinoma who received pancreaticoduodenectomy from June 2012 to December 2017 in the first affiliated hospital of guangxi medical university and the second affiliated hospital of guangxi medical university who met the inclusion criteria were retrospectively analyzed,according to the preoperative peripheral blood routine calculation of patients'NLR value,the median numerical method was used to divide them into the low value group(NRL?2.80)with 42 cases and the high value group(NLR>2.80)with 41 cases,The survival time and outcome of patients in the two groups were calculated during the follow-up period.The survival time was calculated from the date of surgery to the date of death or the deadline of follow-up,To investigate the effect of NLR on the prognosis of duodenal papillary carcinoma.At the same time,Cox proportional risk model was used for multivariate analysis to explore whether NLR is an independent risk factor affecting the postoperative prognosis of duodenal papillary cancer patients.Results1.The 1-year,3-year and 5-year survival rates of patients in the low NLR group were 80.9%,42.8% and 9.5%,respectively,and the median survival time was 32 months.The 1-year survival rate of patients in the high NLR group was58.5%,29.2% and 2.4%,and the median survival time was 27 months.The survival time of the high NLR group was shorter than that of the low NLR group,and the difference between the two groups was statistically significant(P=0.02).2.Univariate analysis was performed and variables with statistical significance(P<0.05)were included in Cox model for multivariate analysis.After correction of age,gender and other influencing factors,the results showed that preoperative high NLR was an independent factor affecting postoperative survival time of duodenal papillary carcinoma patients(P=0.023).conclusion1.Higher preoperative NLR in patients with duodenal papillary cancer may indicate a shorter postoperative survival time.2.NLR is an independent risk factor for postoperative prognosis of patients with duodenal papillary carcinoma.3.NLR=2.80 was used as the cut-off value to predict the prognosis of patients with duodenal papillary cancer,and patients with NLR?2.80 had better prognosis than those with NLR > 2.80.
