Objective To explore the clinical effect of closed negative pressure wound drainage after anterior cruciate ligament surgery.Methods A retrospective analysis was performed on 103 patients who underwent anterior cruciate ligament reconstruction under knee arthroscopy in the same treatment group of the joint surgery department of our hospital from April 2018 to November 2018.The patients were divided into two groups according to whether negative pressure drainage was placed postoperatively.In the drainage group,55 patients were placed with drainage tube and connected to negative pressure suction device.In the non-drainage group,48 patients had no negative pressure suction device.The degree of postoperative knee swelling,knee range of motion,pain score(VAS score),inflammatory index,bleeding status,IKDC score of knee joint 1 and 2 months after surgery,average hospitalization time and other indicators were compared between the two groupsResults 1?The peripheral diameters of the upper thigh of patella in the non-drainage group at 2,3,5 and 7 days after surgery were(42.96±3.25)cm,(43.05±3.22)cm,(42.99±3.16)cm,and(42.77±3.18)cm,respectively;On the2 nd,3rd,5th and 7th days after the operation of the drainage group,thecircumference of the upper thigh of patella was(40.42±3.59)cm,(40.52±3.57)cm,(40.39±3.64)cm,and(40.24±3.64)cm,respectively.The circumference of upper thigh of patella after drainage group was smaller than that of non-drainage group(p <0.05).2?The knee flexion Angle of the non-drainage group and the drainage group was(63.75±6.96)°and(70.38±7.34)°at the 5th postoperative day,respectively.The non-drainage group was smaller than the drainage group,with statistical difference(p=0.001).The knee flexion Angle of the non-drainage group and the drainage group was(86.58±5.50)and(89.15±6.13)at the 7th postoperative day,respectively.The non-drainage group was smaller than the drainage group,with statistical difference(p=0.02).3?The IKDC score of the two groups at 1 month after surgery was 60.00(58.25~61.75)in the non-drainage group and 65(60.00~73.00)in the drainage group,with Z=-2.722 and p=0.1,showing no statistical difference.The IKDC functional scores of the two groups were 72.00(70.00~73.75)in the no drainage group and 72.03(70.75~73.00)in the drainage group,respectively(p=0.746).4?The average hospitalization time in the drainage group was(6.8±1.4)days,while that in the no drainage group was(7.5±5.3)days.There was no statistical difference in the average hospitalization time between the two groups(p >0.05).5?VAS score of the no drainage group was higher than that of the drainage group at 1,3,5 and 7 days after surgery,and the difference was statistically significant(p <0.05).6?The CRP of the no drainage group on the second day after surgery was(21.41±8.41)mg/L,and that of the drainage group was(17.67±6.04)mg/L.The difference was statistically significant(p =0.01).There was no significant difference in esr,WBC and neutrophil percentage between the two groups on the second day after surgery(p>0.05).7?Postoperative hemoglobin decreased(6.9±4.8)g/L in the non-drainage group,and decreased(6.8±5.2)g/L in the drainage group,t=0.1,p=0.91.There was no statistical difference in postoperative hemoglobin decreased between the two groups(p >0.05).Conclusion Closed negative pressure wound drainage after anterior cruciate ligament reconstruction can reduce the pain of affected limbs,reduce the degree of swelling,increase the flexion angle of knee joint after operation,which is beneficial to early rehabilitation exercise.
