Clinical Significance Of OSTA In Premenopausal And Postmenopausal Women Screening For Osteoporosis And Analysis Of Related Factors Of Osteoporosis

Objective This study is aimed to evaluate the Clinical significance of the Osteoporosis Self-assessment Tool for Asians(OSTA)in screening for osteoporosis in the premenopausal and postmenopausal women,and analyze the related factors of osteoporosis.Methods 433 female inpatients or health checkers,40 to 88 years old,who measures bone mineral density by dual-energy X-ray bone resorption method in the first affiliated hospital of guangxi medical university,were selected as the research object.Collecting general data such as Age,weight etc,and bone density report.Besides,biochemical indicators were collected for inpatients.Grouping was based on age,OSTA index,menstrual history,and bone mineral density.Receiver operating characteristics curve(ROC),kappa identity test was used to analyze the effectiveness of OSTA in screening for osteoporosis in different groups women.The related factors of osteoporosis were analyzed by Binary logistic regression analysis.Result(1)When lumbar or hip BMD was used as the diagnostic reference,screening osteoporosis with OSTA?-1,For premenopausal women,the Kappavalue was 0.308(P<0.01),the diagnostic consistency was 87%,The specificity and sensitivity were 97.80% and 25.00%,respectively.For postmenopausal women,Kappa value was 0.460(P<0.01),diagnostic consistency was 73.2%,specificity and sensitivity were 77.60% and 70.5%,respectively.(2)OSTA screen osteoporosis,The area under the ROC curve(AUC)of the groups aged 40-49,50-59,60-69 and 70-88 years old was 0.739,0.800,0.730 and 0.724,respectively,with medium diagnostic accuracy.The optimal diagnostic cut-off points of each age group were 2.45,1.15,-0.5 and-4.7,respectively.(3)For premenopausal and postmenopausal women,when the hip or lumbar vertebra BMD as diagnostic reference,the AUC of OSTA is 0.821,0.829,Respectively,and the optimal cut-off point value,premenopause(1.55)is greater than postmenopause(-0.5).If osteoporosis was defined using hip BMD,the AUC of premenopausal and postmenopausal was 0.757 and 0.829,Respectively.However,when osteoporosis was defined using lumbar spine BMD,AUC before and after menopause was 0.806 and 0.776.(3)Logistic regression analysis analyze related factors of osteoporosis,the results show:OSTA index(P <0.05,OR =0.591),prealbumin(P < 0.05,OR =0.989),Alkaline phosphatase(P <0.05,OR =1.042)are related factors?Conclusion(1)OSTA is not only suitable for postmenopausal women,but also for premenopausal middle-aged women.The accuracy of OSTA screening for osteoporosis is medium,which can effectively screen out high-risk groups of osteoporosis.(2)OSTA is simple,convenient and cost free,it has specific clinical significance in township health centers and community health service institutions to carry out osteoporosis screening.(3)In this study,the related factors of osteoporosis were OSTA index,prealbumin level and alkaline phosphatase level.
