Objective To explore the effect of body weight and body fat on bone mineral density(BMD) in postmenopausal women.To analyze the relationship between bone metabolic index and BMD,and the application of fracture risk factor assessment tools in patients with osteoporosis,and to provide guidance for the early diagnosis and treatment of osteoporosis in postmenopausal women.Methods A total of 131 healthy postmenopausal women,who were had physical examinationsin in the medical center of first affiliated hospital of anhui medical university from May 2014 to August 2014,were selected.Average age is(63.96 7±.60) years old,the natural menopause,and menopause time is more than one ye ar,the average duration of menopause is(14.50 ±8.71) years.The date of age,heig ht,weight,body mass index(BMI),waistline,duration of menopause and mother fract ure history in patients were collected,Ca,P,ALP,25(OH)D3,E2,BGP,TRACP were examed by laboratory method.BMD of the lumbar spine,femoral neck,femur rotor,Wards triangle and body fat were measured by dual energy X-ray bisorptiometry. Combining with the related data,the OSTA index,the fracture probability of the hip(HF) and main parts(MO) in the next 10 years was calculated.The patients were divided into the normal group and the obese group according to BMI 24,c ompare BMD,bone metabolism index and fracture risk probability between group s by t test.The patients were divided into the osteoporosis group and non osteop orosis group,compare body weight,body fat concent,bone metabolism index and f racture risk probability between groups by t test.To explore the relationships bet ween BMD and other effects by correlation analysis.To analyze the identification of osteoporosis evaluated by OSTA and FRAX under the receiver-operating cha racteristic(ROC).Results1.BMD of the lumbar spine,femoral neck,femur rotor of patients in obese group was significantly higher than that in the normal group,but the duration of menop ause in obese group was significantly shorter than that in the normal group(P<0.05).Age,height,BMD of Wards triangle,Vit D,ALP,BGP,TRACP,E2,Ca and P in ob ese group were not significantly different to that in the normal group.2.Age,the duration of menopause,BGP,TRACP,ALP in osteoporosis group were si gnificantly higher than that in the non osteoporosis group,but height,weight,BMI,body fat,E2,Vit D in osteoporosis group were significantly lower than that in the non osteoporosis group(P<0.05).Abdomen circumference,Ca,P in osteoporosis gro up were not significantly different to that in the non osteoporosis group.3.BMI,body fat,weight,OSTA score,E2 were correlated with BMD of the lumbar spine,femoral neck,femur rotor,Wards triangle positively,but age,the duration of m enopause,MO,HF,BGP,TRACP were correlated with BMD of the lumbar spine,fe moral neck,femur rotor,Wards triangle negatively.Waistline,Vit D,ALP,Ca,P were n ot correlated with BMD.4.OSTA score under the Receiver operating characterisitic curve was 0.754, HF and MO under the Receiver operating characterisitic curve were 0.890,0.888.Conclusion1.It is suggested that healty postmenopausal women should increase weight and body fat to prevent osteoporosis.2.It is important for the diagnose and treatment of osteoporosis to monitor the bone metabolism index. 3.OSTA and FRAX tool may be helpful to identify osteoporosis in postmenopausal women.
