ObjectiveTo investigate the prognosis of CK-5/6 and Ki-67 in breast cancer patients by exploring the expression of CK5/6 and Ki-67 in breast tumor tissues and the relationship between various pathological indicators,it can be used for potentil clinical guidance.MethodsA retrospective study is performed involving 142 cases diagnosed as invasive breast cancer from August 2010 to September 2012 in the First Affiliated Hospital of Zhengzhou University.All inpatients were enrolled in the standard: None of the breast cancer patients before the hospital had been treated with radiotherapy,chemotherapy,targeting and endocrine therapy.All patients both were given radical mastectomy and complete cycle of chemotherapy after the examination showed no obvious contraindications for surgery and chemotherapy.Randomly selected 68 patients from 142 breast cancer patients,10 cases of breast fibroadenoma tissuecollected and 12 cases of normal breast tissue were selected in the same period.The expression of CK-5/6 and Ki-67 in 142 cases of breast tumor tissue,10 cases of breast fibroma tissue and 12 cases of normal breast tissue were detected by immunohistochemistry.The connection between CK-5/6,Ki67 and Clinical pathology indexes of breast cancer patients and the influence of these two indexes on the prognosis of breast cancer patients were discussed by using SPSS22.0 statistical software package.Result(1)The positive expression of CK-5/6 was mainly located in the cytoplasm and no expression in the cell membrane.In 68 cases of breast cancer,the positive expression rate of CK-5/6 was 29.4%(20/68),and the positive expression rate of CK-5/6 in 10 cases of breast fibroma was 60.0%(6/10).The positive expression rate of CK-5/6 in 12 cases of normal breast tissue was 91.7%(11/12).There was a significant difference in the expression of CK-5/6 between normal breast tissue,breast fibroma and breast cancer tissues(p<0.001).The positive expression of Ki-67 was mainly located in the nucleus.The positive expression rate of 68 cases of breast cancer was 64.7%(44/68),and there was no positive expression in 12 cases of normal breast tissue and 10 cases of breast fibroma.There are differences in the expression of Ki-67 in breast fibroma,normal breast tissue and breast tumor tissues(p<0.001).(2)In 142 breast cancer patients,there was no statistically significant difference between the expression of CK5/6 and age,pathological type,lymph node status,TNM stage,tumor diameter,histological grade and Her-2 expression status(p>0.05).but there was significant difference between the expression of ER,PR and EGFR(p<0.05).(3)There was no correlation between the expression of Ki-67 and age,pathological type,axillary lymph node status and Her-2 expression status in 142 breast cancer patients(p>0.05),but with TNM stage(p<0.05),tumor diameter(p<0.05),histological grades(p<0.05),ER(p<0.05),PR(p<0.05)and EGFR expression status(p<0.05).(4)CK-5/6 is positively correlated with Ki-67(p=0.009).(5)The expression rate of CK-5/6 in 51 cases of Triple negative breast cancer was 52.9%,and the expression rate of 91 cases of non-triple negative breast cancer was 18.7%,The expression of CK-5/6 was statistically significant between the two types of breast cancer(p<0.001).The expression rate of Ki67 in 51 cases of Triple negative breast cancer was 76.5%,and the expression rate of 91 cases of non-triple negative breast cancer was 52.2%.The difference between the two was statistically significant(P<0.05).(6)In 142 breast cancer patients,Comparison by Log-rank test and Kaplan-Meier graph,the CK5/6 positive expression group had shorter disease-free survival time than the negative expression group,and there was a relationship between CK-5/6 expression and disease-free survival time.Significant difference(p<0.05);the disease-free survival time of patients with low expression of Ki-67 was significantly longer than that of patients with high expression of Ki67.The expression level of Ki-67 was significantly different from that of DFS(p<0.05).The above indicates that Ki-67 is highly expressed and Positive expression of CK-5/6 is an indicator of poor prognosis in breast cancer patients.Conclusion1.The disease-free survival of patients with positive expression of CK-5/6 is shorter than that of CK-5/6 negative expression,CK-5/6 may become an independent prognostic indicator in the future.2.Patients with high Ki-67 expression have shorter disease-free survival than patients with low expression,Ki67 is a sensitive indicator of tumor cell proliferation.3.The expression of Ki-67 in breast cancer patients was different between TNM stage,receptor expression status,axillary lymph node status,tumor diameter and histological grade.The higher the histological grade and the larger the tumor diameter,the higher the expression of Ki-67 in patients;the higher the expression of Ki-67 in patients with negative ER and PR expression.4.The expression of CK-5/6 is correlated with Ki-67.The expression of CK-5/6and Ki-67 in non-triple negative breast cancer is significantly lower than that in riple-negative breast cancer patients,which may be related to metastasis,invasion andprognosis of breast cancer patients.
