Purpose and significance Hypertension is a risk factor for various cardiovascular diseases.Leptin is a protein that is a hormone specifically secreted by adipose tissue containing approximately 146 amino acids.Arteriosclerosis is an angiopathy that is one of the important features in the development of hypertension.Arterial pulse wave velocity(PWV)is a non-invasive marker for assessing arterial stiffness.This study analyzed the serum leptin levels,blood glucose,blood lipids,renal function,and PWV in patients with grade 1,2 and 3 of primary hypertension,and studied the association of these indices with PWV and hypertension classification.It can provide a possible clinical basis for early prediction of cardiovascular disease in hypertensive patients,and further explore the pathogenesis of hypertension and guide the prevention,treatment evaluation and prognosis of hypertension.Study methods The study population consisted of 206 hypertensive patients at all levels from 2012 to 2013 in the hypertension hospital of the First Affiliated Hospital of Guangxi Medical University.Record the age,sex,body mass index(BMI),systolic pressure(SBP),diastolic pressure(DBP),etc.of all selected participants;detect total cholesterol in fasting serum(total cholesterol),TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),homocysteine ??(Hcy),uric acid(UA),creatinine(creatinine,Cr),glycosylated hemoglobin,cystatin C,leptin(LP)and other indicators;and the use of automatic arteriosclerosis instrument to detect PWV values.The t-test,analysis of variance,and analysis of covariance were used to compare and analyze each data indicator.Multiple linear regression(stepwise method)was used to obtain the standardized regression coefficient ? value and standard error.Each factor was assessed with PWV,leptin levels,and hypertension.And its blood pressure grading relationship.Statistical analysis using SPSS 17.0.Results(1)There was no significant difference in age,body mass index,leptin,total cholesterol,triglyceride,LDL,homocysteine,HbA1 c,and arterial pulse wave velocity among patients with grade 1,2 or 3 hypertension.0.05),systolic blood pressure,diastolic blood pressure,creatinine,uric acid,cystatin C were different(P<0.05).(2)Multiple linear regression analysis showed that leptin concentration was significantly positively correlated with hypertension(P<0.05,t=2.288),suggesting that leptin is associated with hypertension.(3)After analysis of covariance,serum leptin levels in hypertensive patients were significantly elevated,and leptin levels were not significantly associated with hypertension blood pressure grading(P>0.05).(4)Multiple linear regression analysis of age,body mass index,systolic blood pressure,diastolic blood pressure,glycated hemoglobin,cystatin C,creatinine,uric acid,total cholesterol,triglycerides,low-density lipoprotein,homocysteine,There was no significant correlation between arterial pulse wave velocity and leptin(p>0.05).(5)Compared with BMI,uric acid and diastolic blood pressure increased with BMI(P<0.05),age,systolic blood pressure,leptin,creatinine,cystatin C,glycosylated hemoglobin,total cholesterol,triglyceride,There was no change in the rate oflow-density lipoprotein,homocysteine,and arterial pulse wave velocity with BMI(P>0.05).(6)Multiple linear regression analysis showed a significant positive correlation between diastolic blood pressure,uric acid and BMI(p<0.05),age,systolic blood pressure,leptin,glycated hemoglobin,cystatin C,creatinine,total cholesterol,triglycerides,No significant correlation was found between low density lipoprotein,homocysteine,arterial pulse wave velocity and BMI(p>0.05).(7)Age,systolic blood pressure,cystatin C,glycated hemoglobin,and homocysteine ??increased with increasing PWV(P<0.05),diastolic blood pressure,body mass index,leptin,creatinine,and PWV.The uric acid,total cholesterol,triglyceride,and low-density lipoprotein did not change with the increase of PWV(P>0.05).(8)By multiple linear regression analysis,age,systolic blood pressure,cystatin C and PWV were significantly positively correlated(p<0.05),leptin,BMI,uric acid,creatinine,total cholesterol,triglycerides,low-density lipoprotein,homotypic half There was no significant correlation between cystine,glycosylated hemoglobin and PWV(p>0.05).Conclusions(1)Serum leptin levels are associated with hypertension and are not significantly associated with blood pressure grading.(2)Age,systolic blood pressure,cystatin C are risk factors for atherosclerosis,and leptin is not a risk factor for atherosclerosis.(3)Diastolic blood pressure and uric acid were associated with BMI,and there was no significant correlation between leptin,systolic blood pressure and BMI.
