Objective1.Through the "doctor-patient communication" follow-up information platform,AIDET communication mode is adopted to follow up type 2 diabetes,providing extended care in terms of diet,exercise,lifestyle,blood glucose monitoring,drugs and disease-related knowledge.2.To evaluate the compliance,satisfaction and blood glucose control compliance rate of discharged type 2 diabetes after follow-up by AIDET communication follow-up mode.3.To explore the clinical value of AIDET communication and follow-up mode,and to provide new nursing ideas for standardized management of dischargedtype 2 diabetes.MethodsUsing convenient sampling method,100 type 2 diabetes discharged from endocrinology department of south hospital of Liuzhou worker's hospital from may 2018 to August 2018 were selected as experimental group.During the same period,100 type 2 diabetes discharged from the endocrinology department of the west hospital of Liuzhou workers hospital served as the control group.The control group was followed up by "doctor-patient communication" combined with traditional communication mode.The observation group was followed up with "doctor-patient communication" and AIDET communication mode.The type 2diabetes patients who were discharged from hospital for 7 days,3 months and 6months were treated by appointment and followed up in outpatient department.Fasting blood glucose,2 hours postprandial blood glucose,glycosylated hemoglobin,renal function,blood lipid,renal function,blood pressure,height and weight of the patients were collected,satisfaction and compliance were observed,and the differences between the follow-up of the experimental group and the control group were compared.Results1.The Effect of AIDET Communication Mode on Blood Glucose: Fasting blood glucose levels in the experimental group were lower than those in the control group for 7 days,3 months and 6 months after discharge,with statistically significant difference(t=4.949,p < 0.001;t=5.751,P <0.001;t=7.032,P<0.001).Seven days after discharge from the experimental group,the blood glucose level was lower than that of the control group at 3months after discharge and 6 months after discharge.The difference was has statistical significant(t=7.773,p < 0.001;t=6.762,P<0.001;t=10.485,P<0.001).2.The Influence of AIDET Communication Mode on Glycosylated Hemoglobin: The levels of glycosylated hemoglobin(HbAIc)in the experimental group at 3 months of discharge and 6 months after discharge were lower than those in the control group,and the difference was statistically significant(t=2.618,P=0.009;t=2.960,P=0.001).The experimental group was discharged.The body mass index(BMI)level at 3 months and 6 months after discharge was lower than that of the control group,and the difference was statistically significant(t=7.194,P=0.008;t=4.942,P=0.027).The experimental group was discharged for 6 months.The blood pressure level was lower than that of the control group,and the difference was statistically significant(t=0.178/0.866,P<0.001).3.The influence of AIDET communication mode on blood lipid and renal function: the triglyceride(TG)in the experimental group was lower than that in the control group at the 3rd month after follow-up,the difference was statistically significant(t=0.599,P=0.001),and the low density lipoprotein(LDL-C)was lower than that in the control group at the 3rd and 6th months after follow-up,the difference was statistically significant(t=1.008,p < 0.001);T=4.942,p < 0.001).there was no significant difference in serum creatinine,urea nitrogen and other indexes between the experimental group and the control group after 3 months of follow-up and discharge(P=0.900)and 6 months of discharge(P=0.909).4.The Influence of AIDET Communication Mode on Satisfaction and Compliance:The two groups of type 2 diabetes were compared between the satisfaction groups after follow-up,and the experimental group was in the third month after the follow-up.After the 6th month,the difference was statistically significant(Z=1.552,P<0.001;Z=0.998,P<0.001).The two groups of type 2diabetes were followed up for follow-up after comparison.After 3 months and 6months after follow-up,the difference was statistically significant(Z=1.618,P<0.001;Z=0.968,P<0.001).Conclusions1.AIDET communication follow-up mode is used for follow-up.Compared with traditional follow-up mode,AIDET communication follow-up mode plays a positive role in reducing fasting blood glucose,2-hour postprandial blood glucose and glycosylated hemoglobin control of type 2 diabetes 3 months and 6 months after discharge from hospital.2.AIDET communication follow-up mode was used for follow-up.The control rate of blood lipid,BMI,blood pressure and other indicators of type 2diabetes 3 months and 6 months after discharge was better than the traditional follow-up mode.3.AIDET communication follow-up mode can improve the compliance and satisfaction of type 2 diabetes after discharge.
