ObjectiveTo investigate the correlation between hip joint damage and bone metabolism in patients with ankylosing spondylitis,and to analyze the risk factors of hip joint injury from the point of view of bone metabolism,so as to provide evidence for predicting the progression of ankylosing spondylitis.MethodThe patients with AS diagnosed in our hospital from May 2013 to December2016 who met the 1984 New York criteria were collected.All patients had completed blood routine,liver function,renal function,coagulation function,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),HLA-B27 gene,parathyroid hormone(PTH),osteocalcin,25-hydroxyvitamin D3,Total type I collagen amino acid and?collagen,pelvic orthotopic DR or sacroiliac joint CT and sacroiliac joint MRI.The patients with imperfect clinical data and other connective tissue diseases,malignant tumors,bone trauma,primary hyperparathyroidism,uremia and so on were excluded.The statistical analysis was carried out by SPSS23.0 software,the quantitative data were tested by two independent samples,the qualitative data were tested by?~2 test,the data that did not conform to the normal distribution were tested by non-parametric test,and the correlation analysis was carried out by Spearman correlation analysis.Result250 cases were collected according to the inclusion criteria,including 2 cases of rheumatoid arthritis,1 case of bone trauma and 14 cases of missing data.101 cases were complicated with hip joint injury(involved group)and 132 cases were not complicated with hip joint injury(control group).The results of t-test of two independent samples showed that there were significant differences in erythrocyte,hemoglobin,erythrocyte distribution width,erythrocyte sedimentation rate and total type 1 collagen amino prolongation peptide between the affected group and the control group.The differences of bone metabolism indexes between the two groups were compared by sex,and there were statistical differences in hemoglobin,erythrocyte distribution width and erythrocyte sedimentation rate between male and female patients,and there were significant differences in hemoglobin,erythrocyte distribution width and erythrocyte sedimentation rate between male and female patients.Osteocalcin,?-collagen special sequence determination,t-PINP also found statistical differences,but there was no statistical difference in these bone metabolism indexes between the two groups.Hip score was negatively correlated with onset age and hemoglobin,and positively correlated with erythrocyte sedimentation rate.ConclusionThe level of ESR in AS patients with hip joint injury was higher than that in AS patients with normal hip joint,the level of erythrocyte sedimentation rate was positively correlated with the degree of hip joint injury,hemoglobin was negatively correlated with the degree of hip joint injury,anemia may be a factor of poor prognosis of hip joint.The level of total collagen type 1 amino prolongation peptide in AS patients with hip joint injury is higher,indicating that bone formation plays an important role in hip joint disease.
