Object Nesfatin-1 is a new anorectic peptide.Studies have shown that under the action of prohormone convertases such as PC3 / 1 and PC2,NUCB2 can be cleaved into three distinct parts: N-terminal nesfatin-1,nesfatin-2 and C-terminal nesfatin-3.Further studies found that the three peptides were injected into the lateral ventricle of mice,and only the mice injected with nesfatin-1 changed their feeding patterns,time,and food intake to inhibit food intake and further change their body weight.Therefore,it was speculated that nesfatin-1 plays an important regulatory role in the feeding mechanism,so it is listed as an anorectic peptide.Subsequent studies suggest that NUCB2/nesfatin-1 is widely distributed in the central and peripheral regions and can play a more comprehensive role through the diffusion of unsaturated transmembrane across the blood-brain barrier,and its extensive colocalization with many other neurotransmitters suggests that the peptide In addition to participating in food regulation,it also participates in other homeostasis function regulation: glucose homeostasis,gastrointestinal function,temperature regulation,cardiovascular function,puberty,and body temperature regulation.NUCB2/nesfatin-1 and insulin co-localize to pancreatic ?-cells,which are released into the blood together with insulin,correct glucose metabolism disorders,regulate insulin resistance,and participate in the occurrence and progression of diabetes.In recent years,the relationship between NUCB2/nesfatin-1 and insulin resistance and progression of diabetes has attracted much attention.However,there are few studies on the serum of NUCB2/nesfatin-1 in the elderly population.This article explores the serum NUCB2/nesfatin in the elderly with different glucose tolerance.The correlation between-1 level and blood glucose and insulin resistance,explored its level of changes in different stages of senile diabetes and related influencing factors,and hope to further explore its research progress.MethodsA total of 80 patients aged ? 60 years old who were admitted to the Department of Geriatric Endocrinology and Endocrinology at the First Affiliated Hospital of Zhengzhou University from July 2017 to January 1818 were randomly selected.According to the 1997 ADA Diabetes Diagnosis and Classification Standard,the glucose tolerance 40 people in the reduced group(FPG<7.0mmol/L,7.8mmol/L?OGTT 2hPG<11.1mmol/L)in the IGT group,newly diagnosed type 2 diabetes group(FPG?7.0mmol/L in T2 DM group,OGTT 2hPG?11.1mmol/L)40 patients with typical hyperglycemia symptoms and randomized serum glucose ?11.1mmol/L),and another 40 patients with physical examination performed 75 g OGTT,FPG<6.1mmol/L,and 2hPG<7.8mmol/L as control group(NGT group).The levels of fasting serum NUCB2/nesfatin-1 were measured by ELISA,and the height,weight,fasting blood glucose(FPG),fasting insulin(FIns),glycosylated hemoglobin(HbA1c),total cholesterol(TC),glycerol were accurately determined.Triester(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),calculation of BMI,insulin resistance index(HOMA-IR),islet ?-cell function index(HOMA-?)).Pearson correlation analysis of the correlation between serum NUCB2/nesfatin-1 and various indicators,multiple linear regression analysis of the factors affecting serum NUCB2 / nesfatin-1 levels.Results1.There was no significant difference in age,BMI,FINS,TG,TC and LDL-C between the three groups(P>0.05).Compared with NGT group,FPG,HbA1 c and HOMA-IR were increased in T2 DM group,HDL-C and HOMA-? were decreased;FPG was increased and HOMA-? was decreased in IGT group(P<0.05).Compared with the IGT group,the FPG and HOMA-IR were increased a nd the HDL-C was decreased in the T2 DM group(P<0.05).2.Compared with NGT group,serum NUCB2/nesfatin-1 levels were increa sed in T2 DM group and IGT group,but there was no significant difference i n serum NUCB2/nesfatin-1 levels between T2 DM group and IGT group(P>0.05).3.Pearson correlation analysis showed that serum NUCB2/nesfatin-1 level s were positively correlated with FPG,HbA1 c,HOMA-IR(r=0.756 r=0.384 r=0.468 P<0.05),and HOMA-??BMIis negatively correlated(r=-0.581 r=-0.156P<0.05).4.Using serum NUCB2/nesfatin-1 level as the dependent variable,age,B MI,FPG,FINS,HbA1 c,TG,TC,HDL-C,LDL-C,HOMA-IR as independent variab les,multiple linear regression analysis showed that FPG(?=0.355 P<0.01)and BMI(?=-0.085 P<0.05)were significant factors affecting serum nesfatin-1,a nd the constant term was 2.369.ConcludesIn the elderly population,serum NUCB2/nesfatin-1 levels increase with the impaired glucose tolerance,and work with insulin to regulate blood glucose levels in the body,and participate in the development of insulin resistance(IR),and its level and fasting blood glucose The level is clearly correlated.
