Study On The Predicament And Countermeasures Of The Implementation Of General Practitioner System

Since the 1980 s began introducing General practitioner system(GPS),our country through a series of trial practice,developed a series of with Chinese characteristics with the GPS system supporting policies,until 2011,issued the state council on establishing a GPS guidance system "policy documents,officially opened the general system construction within the scope of work throughout the country.The general practitioner system is implemented in more than 50 countries around the world,and plays an indelible role in the medical system of each country.It plays a role in the health of residents but ACTS as the "gatekeeper".However,in the current medical system of China,no matter in the hierarchical diagnosis and treatment system with Chinese characteristics or in the construction of the grass-roots medical and health system,the advantages of the general practitioner system itself have not been shown.This article embarks from the GPS system,by analyzing development course and current situation of the development of GPS system,from the China health statistics yearbook,"China's health and family planning statistics bulletin of the career development as well as provincial health statistics data to describe the general practitioners practice situation and difficulty analysis,from a zero-sum game,information asymmetry and incentive compatibility theory to analyze the cause of the plight of system building,and advanced experience of foreign system of general practitioners and apply the above theory to build the new GPS system,in order to promote the GPS system to use" first option at the grass-roots level ",Give full play to the role of general practice system in allocating medical resources,and finally realize the "first diagnosis system" and medical fee control function.In this paper,through a zero-sum game,the information is wrong,the theory of incentive compatibility and so on to analyze the GPS system exists in the system design is not reasonable,and attempts to build a new GPS system,from the following several aspects: one is the general practitioner in the medical insurance management system in our country,break traditional ascription problem of doctors,such powerful doctors can have enough decision-making power,thus really do "first option at the grass-roots level.Second,it is necessary to establish and improve supporting policies for the system of general practitioners in China,improve and standardize the relationship between the rights and obligations of general practitioners and residents' fingers from the perspective of law,and establish a salary system for general practitioners with positive incentives.Third,establish a new medical insurance payment system through the system of general practitioners,and truly realize the medical fee control function of general practitioners through medical insurance payment,so as to change the wrong guidance of seeking medical treatment in the current medical insurance system of China,which is "treat major diseases and ignore minor diseases".Fourth,establish the competitive elimination mechanism of general practitioners,develop the system of general practitioners into a marketoriented mechanism of "in and out",so as to promote general practitioners to improve their medical level and improve residents' satisfaction with medical treatment.Fifth,the use of artificial intelligence Internet technology to improve the service level of general practitioners,the use of modern science and technology,the establishment of health records of residents,the real realization of medical resources sharing,and ultimately promote the optimal allocation of medical resources.
