Objective:To explore the correlation of the incidence and social characteristics in patients with osteoporotic vertebral compression fractures(OVCFs),and to provide certain evidence for prevention,dignosis and treatment of the disease.Method:Data between January 1,2009 to December 31,2018 were collected from electronic medical documents of the spine department and a total of 19700 cases of spine-related diseases were counted,including 2028 patients with OVCF accounting for 10.29%of the total number of hospitalized patients in spinal surgery.The statistics includes gender,age,birth-place,nation,profession,injured segment,concomitant disease and diagnosis and treatment methods of patients who suffered from OVCFs.Descriptive statisics,t-test,Chi-square test,Kruskal-Wallis or Wilcoxon test and Spearman correlation analysis were used to analyze the correlation of demographic and clinical characteristics of patients and the incidence of OVCF.Results:1.In 2009-2018,the number and composition ratio of OVCF increased year by year,with 374 males(18.44%)and 1654 females(81.56%).The male-female ratio was close to 1:4.4,and the number of females were apparently more than that of males.The average age is(70.50±9.73)years old and the average age of men is(73.29±10.23)while the average age of women is(69.87±9.46);there is significant difference between the mean age of OVCF patients with different genders(t=6.2218,P<0.05).In addition,the average age of OVCF patients in 2009-2018 is increasing year by year.2.There were significant differences in age distribution of OVCFs in different gender(?2=:98.91,P<0.05).When the age was less than 70 years old,the fracture ratio of female was higher than that of males,especially between 60 and 70 years old(38.63%:17.91%);However,when the age was more than 70 years,the fracture ratio of male is higher than that of female,especially after more than 80 years old(29.15%:12.70%).3.There were significant differences in occupation distribution of OVCFs in different gender(?2=34.96,P<0.05).Peasants(47.94%),retired staffs(18.32%)and workers(14.63%)are the top three in female while peasants(35.56%),workers(26.47%)and retired staffs(20.05%)in male.4.According to statistics,the proportion of OVCF patients who came to our hospital in winter(December-February)was the highest(31.51%),and that in spring(March-May)was the least(17.70%).The other two quarters were not much different There were no significant differences in seasonal distribution of OVCFs in different gender(?2=7.98,P>0.05).5.The causes of injury were divided into falls,car accidents,sprains,drops and others.It was found that the fall accounted for 77.12%,which was the main cause of OVCF.There were statistical dfferences in the distribution of OVCF injury in different genders(?2=11.45,P<0.05).6.The concomitant diseases were divided into hypertension,spinal degenerative diseases(SDD),diabetes,coronary heart disease,knee and hip osteoarthritis,cerebral infarction and others,and hypertension(31.36%),SDDs(21.22%)and diabetes(16.43%)are the top three concomitant diseases.Statistical Analysis by Chi-square test found that there was no significant difference between the gender and each accompanying disease(P<0.05).7.There were significant differences in the distribution of the number of fracture vertebral bodies of OVCF patients of different ages,with fracture history and without fracture history,with smoking history and without smoking history,menopause before age 50 and menopause after age 50(P<0.05).However,there was no significant difference in the number of fracture vertebral bodies between drinking history and no drinking history(P>0.05).Among them,Spearman correlation analysis showed that the number of fractured vertebral bodies was positively correlated with age(=0.79,P<0.05).The older the age,the more the average number of fractured vertebral bodies.8.The number of patients who adopt bone mineral density(BMD)test and bone turnover maker test were 118 and 106 before 2014 while the number were 568 and 837 after 2014 and the differences were statistically significant(P<0.05).1837 hospitalized patients underwent surgical treatment,in which PKP and PVP accounted for 88.35%Conclusion:1.The proportion of OVCFs in spine diseases and average age of patients in the period of 2009-2018 is increasing gradually in Qilu Hospital of Shandong University.2.Distribution of the age,occupation and cause of injury are related to the gender,but there is no correlation between distribution of season,fracture segment and comorbidities and gender.3.The age,previous fracture history,female menopausal age and smoking history are correlated with the number of fractured vertebral bodies while drinking history was not related.4.Preoperative bone density examination is the gold standard for the diagnosis of osteoporosis and bone turnover marker is an important reference forjudging the type of osteoporosis and medication guidance.In recent years,more and more attention has been paid to the preoperative bone density examination and bone turnover marker test in the diagnosis and treatment of OVCF.Interventions and educations in these high risk populations are important to decrease the incidence of OVCFs.
