Background:Osteoporotic fracturesis known as brittle fracture or low-energy fracture,which usually based on osteoporosis,due to decreased bone strength,resulted of slight trauma or non-injury factors in the daily life.The common parts of osteoporotic fractures are usually taking place in the thoracic and lumbar segments of the spine,the hip and the wrist.It is the most severe osteoporotic fracture,as the high rate of disability and mortality of the osteoporotic hip fracture.According to the research,there are about 50%of the patients underwent long-term disfunction,including chronic pain,functional strength weakening,auxiliary equipment and decrease of function.Osteoporotic fracture of vertebral can lead to chronic back pain and vertebral kyphosis,followed by circulatory,respiratory and digestive function declining,even losing the self-care ability.All these will seriously affect the quality of life of the elderly,and have become serious problems faced by patients,family members and clinicians.With the aggravating trend of aging population,the number of osteoporosis and osteoporotic fractures is enormous.It causes great harm to patients themselves,takes up a lot of medical resources,and also brings heavy burden to the whole society.Object:Osteoporotic fracture is a common disease in clinic.It is one of the severe factors of threating the health and life of the elderly,as the high disability rate and high mortality rate.How to reduce the incidence of such diseases is a difficult problem for clinicians.It is the key to solve these problems by further researching the occurrence and development of osteoporotic fractures.This study analyzes the clinical data of patients in Qilu Hospital of Shandong University in the year of 2012-2016.This research studies the characteristics of clinical epidemiology,osteoporotic fracture risk factors of hip and vertebral fractures and treatment cost etc.Methods:This research collects the clinical data of hospitalized patients with osteoporotic hip or vertebral fracture,which treated for the first time in Qilu Hospital of Shandong University in the year of 2012-2016.In the research of the distribution of the incidence of hip fractures and vertebral fracture,which uses different test methods according to the design of hypothesis test,including linear regression and t test method.In the study of the treatment cost,this paper mainly adopts the method of panel regression.The patients were divided into ten groups according to the part of fracture and the age group,and coded in each group.The hospitalization days and hospitalization expenses were taken as dependent variables,and smoking,drinking,complications,treatment methods and other factors were taken as independent variables.Results:(1)We collected 1387 samples of osteoporotic fractures of hip or vertebral,including 359 men(26%),1028 women(74%),the ratio of men to women was 0.35:1.In the distribution of fracture part,633 cases of hip and 754 cases of vertebral.Only 596 cases(42.9%)had anti-osteoporosis treatment in the medical orders.(2)The results of osteoporotic fracture and age study showed that the number of ratio of hip osteoporotic fractures increased with age(r=0.156>0,p=0.017).the number of ratio of vertebral osteoporotic fractures decreased with age(r=-0.156>0,p=0.017).(3)The incidence of hip fracture,the increase rate of female incidence rate is higher than the increase rate of male incidence rate with age increasing(r=0.049>0,p=0.015).In vertebral fractures,the increase rate of female incidence rate is lower than the increase rate of male incidence rate with age increasing over 60 years old(r=-0.076<0,p=0.068).(4)There is no significant difference in the seasonal distribution of hip or vertebral osteoporotic fracture(X2=0.02,df=3,P=0.999),(X2=0.002,df=3,P=0.999).(5)Fall is the main injury mechanism of hip osteoporotic fracture accounting for 84%,which is significantly higher than that of spontaneous fractures(T>0,p=0.999).While vertebral osteoporotic fractures are mostly spontaneous fractures(56%)(T<0,p=0.907).(6)The average hospitalization days of the patients with hip osteoporotic fractures were 12.8 days,and the average hospitalization cost was 48728.71 yuan.The average hospitalization days of patients with vertebral osteoporotic fractures were 6.3 days,and the average hospitalization cost was 28987.51 yuan.(7)There is a significant positive correlation between the cost of osteoporotic fracture and the number of complications(a=1475***)?Drinking habits(?=8199***),and the cost of surgical treatment(a=33524***)will also be higher.The hospitalization days of osteoporotic fractures also showed a significant positive correlation with the number of complications(a=0.74***)?Smoking habits(?= 1.51***)and surgical treatment(a=1.94***).Conclusion:(1)Age is an important factor affecting the incidence of osteoporotic fractures.Elderly people with osteoporosis should take active measures to prevent fracture.(2)The days and expenses of hospitalization of hip fractures were significantly higher than those of vertebral fractures.(3)Fall is the main inducement factor of osteoporotic hip fracture.People with osteoporosis should prevent fall.(4)In the cost of treatment,the number of complications and surgical treatment are important factors for the improvement of the cost of osteoporotic fracture treatment.Meanwhile,the bad habits of smoking and drinking have an important influence on the cost of the treatment of osteoporotic fracture.(5)Clinicians still lack awareness of treating osteoporosis.
