Risk Factors Of Adhesive Extrusion Bleeding After Injection Of Gastric Variceal Tissue Adhesive

Objective There are many patients with bleeding after endoscopic tissue adhesive injection in clinic.The general data and some laboratory tests of patients with cirrhotic gastric variceal bleeding or secondary prevention of tissue adhesive injection were conducted to analyze the factors affecting the occurrence of adhesive extrusion bleeding after tissue adhesive injection.We collected these data so as to provide reference basis for reducing adhesive extrusion bleeding clinically.Methods We collected the patients with tissue adhesive injection for stopping cirrhotic gastric variceal bleeding or secondary prevention in Shandong Provincial Hospital Digestive Department from 2010 to 2017 and we made the follow-up time one year.Then we divided these patients into two groups including 33 cases of adhesive extrusion bleeding group and122 cases of non-bleeding group based on whether they have adhesive extrusion bleeding or not.We compared the differences between the two groups in factors of gender,age,smoking history,history of drinking,presence of ascites,Child-Pugh,type of gastric varices,platelet count,neutrophil percentage,albumin,alanine transaminase,aspartate transaminase,gamma glutamyl transpeptidase,total bilirubin,creatinine,serum sodium,prothrombin time,activated partial thromboplastin time.We performed non-conditional logistic regression analysis on the significantly different factors to identify the independent risk factors for adhesive extrusion bleeding and recorded related risk factors.Results The difference of age,neuter percentage,glutamic transaminase and prothrombin time between the two groups were statistically significant,and prothrombin time was positively correlated with the occurrence of adhesive extrusion bleeding.The bleeding time was mainly within 1 month after tissue adhesive injection.Conclusions We found that prolongation of prothrombin time is an independent factor for the occurrence of adhesive extrusion bleeding and the highest incidence of bleeding occurred within 1 month.
