Objective:Illustrate the change in the process of gastric cancer H.pylori infection and the correlation of the expression of CD44 and apoptosis regulation genes Survivin and possible mechanism of action,and in combination with clinical data,further explore these genes in the early diagnosis of gastric cancer,metastasis and the role of prognostic judgment,in order to provide early diagnosis and treatment of gastric cancer has the reference value of molecular biology.Methods: Urea enzymatic and carbon 14 method with rapid detection of 50 cases of chronic non-atrophic gastritis 50 patients with intestinal metaplasia and 50 patients with chronic atrophic gastritis,50 cases of special-shaped hyperplasia(AH),50 cases of gastric cancer(GC)in the organization of H pylori infection,using immunohistochemical testing specimens respectively in the expression of CD44 and Survivin protein,in the process of the analysis of H pylori infection in gastric cancer and then there is a certain relationship between protein expression and protein expression with apoptosis;Furthermore,the expression levels of CD44 and Survivin-mRNA in samples were detected by real-time fluorescence PCR.Combined with the pathological features of clinical cases,the role of CD44 and Survivin in the early diagnosis,invasion,metastasis,and prognosis of gastric cancer was discussed.Result:(1)the infection rate of H pylori increased significantly with the aggravation of the malignant degree in the process of CNAG CAG IM AH GC,which was 10%,24%,44%,68%,80%,respectively.The infection rate of H pylori in the CAG,IM,AH,and AC comparing the differences between the two groups,the results show that the two groups have statistical significance.(P <0.05).The infection rate of H pylori in the GC group was higher than that in the CAG group(P < 0.05).During the development of CNAG CAG IM AH GC,the positiveexpression rate of Survivin protein gradually increased with the severity of the the the lesion,which was 0%,8%,18%,58%,68%,respectively.The positive rate of CNAG was increased in the AH group(P < 0.01),and was significantly higher in the GC group than in the CAG and IM groups(P < 0.05).The positive expression rate of CD44 protein increased gradually with the aggravation of the lesion,which was 4%,16%,32%,62% and 80%,respectively.The GC group was significantly higher than the CAG and IM groups(P <0.01).The positive expression rates of CNAG-CAG-IM-AH-GC were 0,20 %,40%,58%,72%,respectively.The positive expression rates of CD44 were 4%,16%,36%,64%and 76%,The expression level of survival stachytene was 0,0.068,0.007,0.347,0.060,0.553,0.080,1.646,0.356,respectively,in the IM,AH and GC groups,compared with the CAG group(P<0.05,P<0.01),the AH group,compared with GC group,IM group had significant difference in IM group(P < 0 05),and there was significant difference between IM group and IM group(P < 0 05).The difference was statistically significant(P<0.01).The expression levels of CD44-mRNA were 0.095 0.015,0.115 0.012,0.173 0.027,0.224 0.042,0.368 0.036,respectively.Compared with CNAG and CAG groups,the mRNA levels in the IM,AH and GC groups were significantly higher(P<0.01),the mRNA the expression level of the other group was higher than that of the AH group.in the IM and AH groups(P<0.01).(4)positive expressions of Survivin in patients with H pylori infection were higher in CAG,IM,AH and GC groups than in patients without H pylori infection,and the difference was statistically significant(P<0.05).The positive expression rate of CD44 in the CAG,IM,AH and GC groups was higher than that in the negative group(P<0.01).The positive expression rate of CD44 in H pylori positive group compared with Hpylori negative group,the expression level of survivin and CD 44 protein in CNAG group was higher than that in VEGF negative group,and there was no significant difference between the two groups.There was a certain correlation between survivin and CD 44 protein expression level.in patients with highly differentiated,moderately differentiated,poorly differentiated and undifferentiated gastric cancer showed a gradually increasing trend,with Survivin rate of 57.69%,75%,93.75%,CD44 rate of 61.54%,87.50%,93.75%,respectively.Survivin and CD44 protein expression rates were significantly higher in the lymph node metastasis group than in the non-lymph node metastasis group(P<0.05).Patients in the group with distant metastasiswere higher than those in the group without distant metastasis(P<0.05).The correlation coefficient of Survivin and CD44 expression in gastric cancer patients was r=0.901(P <0.001).Conclusion:(1)from the protein level and cell level analysis of Survivin and CD44 gene in gastric mucosal canceration(CNAG to the CAG,IM-AH-GC),the changing law shows the Survivin and CD44 expression level increased,the corresponding mediated by Survivin apoptosis inhibition enhanced gradually,CD44 adhesive ability strengthen gradually,thus mediated tumor cell invasion and metastasis,eventually leading to the onset of cancer of the stomach;(2)studies on the role of H pylori infection in the regulation of Survivin and CD44 in gastric mucosal cancer showed that H pylori infection up-regulated the expression of Survivin and CD44-mRNA,and the expression level of Survivin and CD44 protein increased,suggesting that the imbalance in the regulation of Survivin and CD44 genes caused by H pylori infection may be one of the mechanisms of gastric cancer.(3)clinical analysis shows that Survivin and CD44 expression are closely mainly influenced by forest farm performance and biological behavior.of gastric cancer,and have an important influence on the prognosis of gastric cancer patients.
