The Clinical Study Of YiQiXuGu Mixture Liquid Combined With PRP In The Treatment Of Kellgren-Lawrence??? Knee OA

Objective: To investigate the clinical efficacy of Yi Qi Xu Gu mixture liquid combined with intra-articular injection of autologous platelet-rich plasma(PRP)in the treatment of Kellgren-Lawrence I-III knee OA.Methods:Forty-six patients with Kellgren-Lawrence I-III knee OA who underwent orthopedics in our hospital from December 2017 to February 2019,including 22 males and 24 females,with a minimum age of 46 years and a maximum age of 70 years,were selected.Forty-six eligible patients were randomly divided into observation group and control group.Among them,23 were in the observation group and 23 in the control group.During the observation and treatment,due to imperfect treatment cycle,incomplete data,and regular follow-up,40 cases of knee OA patients were collected for study.The observation group and the control group were each 20 cases.The observation group used Shandong Weigao limited.The company provided a disposable PRP preparation kit for autologous PRP collection and prepared 3.5ml of PRP.The surgeon then used the conventional knee joint injection method to prepare the 3.5ml PRP for intra-articular injection from the tibia.The upper edge of the needle was pushed into the joint cavity,and each injection was performed once every 2 weeks and 4weeks,a total of 3 times.At the same time,30 ml of Yi Qi Xu Gu mixture liquid was started from the injection of PRP,3 times a day for 4 weeks.The control group was treated with conventional intra-articular injection of sodium hyaluronate.For the first time,sodium hyaluronate injection(Spitter 2ml: 20 mg Shandong Bausch & Lomba Pharmaceutical Co.,Ltd.)+ triamcinolone acetonide injection(1ml: 40 mg Kunming Jida Pharmaceutical Co.,Ltd.)a total of 3ml of conventional knee joint injection,and every other week only injection of sodium hyaluronate injection,routine injection treatment a total of 5 times;two groups of patients before treatment and after treatment 1 month,3 In the month and June,the knee joint HSS score and the cartilage injury repair MOCART scoring system were used to compare and analyze the therapeutic effect.Results:1.Overall efficacy There was no significant difference in the short-term efficacy between the observation group and the control group at 1 month and 3 months after treatment,and the long-term efficacy observation group(Yi Qi Xu Gu mixture liquid combined with PRP group)after treatment was better than the control group(sodium hyaluronate group)has an advantage.2.Hss evaluation efficacy After treatment,the two groups of patients in the observation group and the control group had no significant difference in the recent HSS scores in the first month and the third month after treatment(P>0.05).The long-term effect of the 6 months after treatment,the observation group(Yi Qi Xu Gu mixture liquid combined with PRP group)compared with the control group(sodium hyaluronate group)in the improvement of patients with knee function(P <0.01),there was significant statistical difference.Significance;there was a statistically significant difference in relieving knee pain and improving knee mobility(P<0.05);and improving knee muscle strength,flexion contracture,stability,and reduction items(P>0.05).The difference was not statistically significant,indicating that the efficacy of the observation group(Yi Qi Xu Gu mixture liquid combined with PRP group)in the 6 months after treatment was superior to that of the control group(sodium hyaluronate group).3.Mocart score evaluation efficacy The difference between the two groups of patients in the 1 month and 3 months after treatment in the nuclear magnetic MOCART score(P>0.05)was not statistically significant,indicating that the two in the repair of knee OA degenerative cartilage effect,after treatment The nuclear magnetic performance was not obvious in January and 3 months after treatment;however,there was a statistically significant difference between the two groups in the MOCART score(P<0.01)after treatment,indicating that the observation group(Yi Qi Xu Gu mixture liquid combined with PRP group)Compared with the control group(sodium hyaluronate group),the observation group had an advantage in the nuclear magnetic MOCART score,indicating that the former has a stronger ability to repair knee OA degenerative cartilage than the latter.4.Safety indicators During the observation group,there were 2 cases of post-injection pain and swelling aggravation.After active intervention with ice,rest and other interventions,the patients gradually relieved after 2 days,while the control group had no other adverse reactions.Conclusion: Yi Qi Xu Gu mixture liquid combined with PRP has a good effect on knee OA,especially the long-term effect after treatment is more significant,and it is effective in improving the pain,function and activity of patients with knee OA.However,because PRP needs to be injected 3 times,it is not reimbursable for self-funded projects,the cost is relatively high,the patient's economic burden is large,and the patient acceptance is different,which makes the technology development more difficult.However,because it has a certain role in repairing the degenerative cartilage of the knee joint,it can be prospectively expected to slow the progression of the disease in patients with knee OA or to make it in a stable plateau stage,then it can reduce the long-term surgery of patients with knee OA.Interventions such as the possibility of joint replacement have important and practical implications for the patient.
