ObjectiveThrough the analysis of the clinical and imaging features of cementoblastoma,to improve the clinical doctors' understanding of cementoblastoma,and to provide support of the clinical diagnosis and treatment plan.To explore the imaging differential diagnosis of cementoblastoma.MethodTwenty patients with cementoblastoma who were admitted to Shandong University Stomatological Hospital and Qilu Hospital of Shandong University from July 2011 to January 2019 were collected.The clinical and imaging data were statistically analyzed.Use the imaging features obtained from the analysis to distinguish it from other diseases.Results(1).The 20 patients in this group were 7 to 69 years old,with an average age of 36.70±15.22 years,there were 9 males and 11 females.Among them,there were 2 patients(10%)aged 7-20 years old,4 patients(20%)aged 20?30 years old,8 patients(40%)aged 30?40 years old,1 patients(5%)aged 40?50 years old and 5 cases(40%)over 50 years old,patients who were 20?40 years old accounted for 60%.(2)?The 20 patients in this group had a disease course of 3 months to 20 years,of which 11 patients(55%)were within 4 months,3 patients(15%)were 4 to 6 months,1 patients were 6 months to lyear(5%),2 case(10%)in 1?3 years,2 case(10%)in 3?10 years,1 patient(5%)over 10 years,and 75%patients' course were in 1 year.(3)?In this group,2 patients(10%)occurred in the maxilla,18 patients(90%)occurred in the mandible;2 patient in the mixed dentition(10%),18 patient in the permanent dentition(90%);3 patients in the anterior-tooth area(15%),17 patients(85%)in the posterior-tooth area.Of the 20 patients,2 involved impacted teeth and 3 involved multiple teeth.The affected teeth are all vital teeth.(4)?In this group,there were 5cases of Jaw swelling,7cases of pain,6 case of jaw swelling along with pain,2 cases of asymptomatic.Within the 18 cases of Jaw swelling,pain and Jaw swelling accompanied by pain,there were 1 cases of infection,and 2 case of lower lip numbness.(5)?In this group,2 patients underwent simple tumor removal and continued observation of the affected teeth.3 patients underwent tumor extraction plus root canal therapy,12 patients underwent tumor removal plus extraction of affected teeth,and 2 underwent tumor removal plus mandible partial resection,1 case underwent tumor removal plus iliac bone grafting plus jaw osteoplasty.17 of the 20 patients were followed up and only 1 patient had recurrence.(6)?In this group of patients,the X-ray appears as a block-block image with clear boundaries and uniform or nonuniform density,surrounded by narrow low-density shadows,all connected with the root of the affected tooth.In this group,the tumor size measured from the X-ray was 3x2 cm at the maximum,and the minimum was 0.8×0.8 cm,with an average of 2×1.5 cm.There were 19 patients with single block shadow and 1 patient with multiple block shadow;There were 18 patients of high-density uniformity in the center of the tumor,2 patients of unevenness,and 20 patients of tumor margins were all low-density images.Conclusion(1)?Cementoblastoma mostly occur in 20 to 40 years old,the course of disease is usually less than 1 year,and there is no significant gender differences,mostly occurs in the mandibular posterior-tooth region,and mostly occurs in permanent dentition.(2)?The clinical manifestations of cementoblastoma are mainly swelling of the jaw bone,pain and swelling of the jaw with pain,accompanied by symptoms such as infection,numbness,etc.and the early lesions are mostly asymptomatic.(3)?The most common surgical treatment for cementoblastoma is tumor removal plus the extraction of the affected tooth.According to the patient's needs and pathogenetic condition,tumor resection plus root canal treatment or simple mass resection can be performed.(4)?Imaging examination is an important method for the diagnosis of cementoblastoma.Its characteristic imaging findings contribute to the differentiation diagnosis to the disease from idiopathic osteosclerosis?cementum-ossifying fibroma and cementum dysplasia.
