ObjectiveTo discuss the clinical,imaging,preoperative diagnosis,pathology and treatment of bronchogenic cyst in adult.Via analyzing the clinical data,we attempt to improve the diagnosis.MethodsWe retrospectively reviewed24patients admitted to our hospital with histopathologically proved bronchogenic cyst from July2009to April2014.ResultsAmong the patients,there were10males and14were females. The mean age was47.3(range,24to76) years.9patients presented with cough and sputum,5with short breath,4with chest pain,4with fever,6patients were asymptomatic. According to the imaging,9patients were found in the pulmonary parenchyma,15in mediastinum.The imaging feature in computer tomography (CT) was characterized as a round, well circumscribed, homogeneous water-density or soft tissue or thin walled cavity, the cysts were no enhanced or little enhanced with contrast enhancement. If secondary infection was presented, an air-fluid level could be seen. Magnetic resonance imaging(MRI) was performed in7patients. Most cysts showed low or moderate signal intensity on T1-weighted MR images and high signal intensity on T2-weighted MR images. Only7patients were diagnosed as bronchogenic cyst preoperative,6were misdiagnosed as lung cancer,3were misdiagnosed as neurogenic tumor,2were misdiagnosed as thymoma,1was misdiagnosed as tuberculosis,1was misdiagnosed as pulmonary abscess and1was misdiagnosed as bullae of lung.Pseudostratified ciliated columnar epithelium,gland,cartilage,elastic fiber and smooth muscle were all confirmed by light microscopy.Thoracotomy was performed in14patients, while thoracoscopy was performed in10patients. The mean follow-up period was36.6(range,5to62) months, during the follow-up periods no recurrence was detected.ConclusionsThe main clinical feature of bronchogenic cyst in adult was cough, sputum and short breath. Computer tomography(CT) and Magnetic resonance imaging (MRI) were helpful for diagnosis. The lesion was easily misdiagnosed as other diseases because of lack of apparent clinical manifestation and imaging feature.Surgical resection was the best way for diagnosis and treatment of bronchogenic cyst.
