objective:Obstructive sleep apnea-hypopnea syndrome(OSAHS)is a common sleep disorder in which upper airway injury and collapse are one of the common causes of morbidity in patients with neuromuscular insufficiency of the upper airway.The genioglossus is Changes in the myoelectrical activity of the major muscles of the upper airway affect the severity of OSAHS disease.At the same time,patients repeatedly appear during sleep apnea and hypoxia caused by intermittent hypoxemia,hypercapnia and other body can produce a series of systemic hazards,while affecting the secretion of multiple hormones.This topic by observing OSAHS female patients and healthy adult women overnight polysomnography monitoring data,including EEG,ECG,genioglossus myoelectricity,oxygen saturation,sleep apnea-hypopnea index(AHI)and a series of Data to explore the relationship between genioglossus EMG activity and serum sex hormone levels in OSAHS female patients and to study the effect of genioglossus and sex hormones on the incidence of OSHAS female patients.methods:From March 2017 to November 2017,30 cases of OSHAS patients diagnosed by PSG from Shandong Provincial Hospital Sleep Monitoring Center and orthodontics department were selected as the observation group and were selected from the physical examination of Shandong Provincial Hospital 30 cases as control group.Subjects were women aged 25-60 years.Serum testosterone,estradiol,progesterone and prolactin levels were measured by radioimmunoassay(RIA).Sleep monitoring equipment was used to conduct overnight polysomnography(PSG)to record sleep data of all groups,including awakening index,average Oxygen saturation,the lowest oxygen saturation,the average heart rate,AHI index,polysomnography at the same time in the acquisition of genioglossal muscle.Mainly from the following aspects of analysis and comparison:(1)two groups of subjects sleep monitoring related indicators comparison.(2)Comparison of sex hormones between the two groups.(3)Comparison of myoelectric activity of genioglossus between two groups.(4)the correlation between the data indicators.This study uses SPSS20.0 software for statistical analysis.Measurement data with x ± s that independent sample t test and non-parametric test analysis of the differences between the two groups.Pearson correlation was used to analyze the correlation of each group of indicators.P<0.05 for the difference was statistically significant.Results:(1)There were significant differences between the two groups in the main sleep monitoring indicators,including AHI index,awakening index,sleep efficiency,average oxygen saturation and minimum oxygen saturation(P<0.01).(2)(2)There were significant differences in serum levels of testosterone(TO),estradiol(E2),progesterone(PRG)between the two groups(P<0.01),and the levels of the three hormones in the observation group were significantly lower than those in the control group.There was no statistical difference in the serum levels of prolactin in the two groups(p>0.05).(3)Comparing the electromyographic activity of genioglossal muscles,the observation group was significantly lower in stage REM than the control group(P<0.01),and significantly higher in deep sleep and awakening stage than the control group(P<0.01).(4)The correlation analysis of the main experimental indicators of the two groups,Serum testosterone levels were negatively correlated with AHI index(p<0.05),progesterone levels were related to AHI index and minimum oxygen satLuration(p<0.05),AHI index,minimum oxygen saturation were significantly correlated with the electromyographic activity of genioglossal muscles in N3,REM and waking periods(P<0.01);testosterone level was correlated with the electromyographic activity of NREM during sleep(P<0.05),and with N1,N2 and REM muscles.The change rate of electromyographic activity was correlated(P<0.05);the level of estradiol was correlated with the change rate of electromyographic activity in stage N1,N2 and REM(P<0.05);the level of progesterone was significantly correlated with the change rate of electromyographic activity in stage N3(P<0,01),and was correlated with the change rate of electromyographic activity in REM(P<0.05).Conclusion:The levels of serum testosterone,progesterone and estrogen decreased in OSAHS female patients,and serum testosterone and progesterone levels could reflect the severity of OSAHS.In OSAHS female patients,the myoelectric activity of genioglossal decreased obviously in stage REM,increased during sobriety period,and the degree of change could reflect the severity of OSAHS.The level of sex hormone in female patients with OSAHS was correlated with the myoelectric activity of genioglossal:The testosterone level was correlated with NREM period electromyography,the progesterone level was significantly correlated with the stage N3 electromyography,and the levels of testosterone,progesterone and estrogen were correlated with the change rate of genioglossal electromyography in stage REM.
