Objective:To study the incidence of Thyroid nodules(TN)in patients with systemic lupus erythematosus(SLE),and to explore its clinical characteristics in order to provide a clinical reference.Methods:A retrospective study was conducted on 145 SLE patients admitted to Rheumatology Department of 967 Hospital of PLA Joint Logistics Support Base from December,2013 to December,2017.141 patients were female 96.9%and 4 patients were male 3.1%with an average age of(37.90?3.97).The basic clinical data of the patients included sex,age,weight,course,history,blood routine,fasting blood glucose(FBG),ESR,blood lipid,function of thyroid,liver and kidney,quantification of 24-hour urinary protein,immunoglobulin(IgA,IgG,IgM),complement(C3,C4).The patients were divided into TN group and non-TN group based on the results of color Doppler ultrasonography.Furthermore,the incidence of thyroid dysfunction and other clinical indicators were analyzed and made an identification between the two groups.Statistical methods were analyzed by T test,rank sum test and X~2 test.Results:1.145 patients with SLE,95 patients 65.5%were enrolled in TN group,94patients were women,with an average age of(40.73?14.30)years and an average course of(96.85?62.26)months;There are 50 patients 34.5%in non-TN group,47patients were women,with an average age of(32.52?12.29)years and an average course of(78.56?49.34)months.The age of patients in TN group was significantly higher than that in non-TN group(P<0.01),but weight and course between the two groups were no significant difference(P>0.05).2.95 SLEs in TN group were grouped according to age,and the incidence of TN was 56.3%when the age was less than 20years old.The incidence of TN in 60-70 years old was the highest,accounting for 90%.The analysis showed that the incidence of SLE with TN increased with age(X~2=10.992,P<0.01).3.77 out of 95 TN patients were examined for thyroid function,of which 21(27.3%)had abnormal thyroid function,and subclinical hypothyroidism(SCH)was the most common(47.6%).Among the 32 non-TN patients,only 1(3.1%)had abnormal thyroid function.The incidence of thyroid dysfunction in TN group was significantly higher than that in non-TN group(P<0.05).4.Comparing the difference of thyroid hormone level,it was found that there was significant difference in thyroid peroxidase antibody(TPOAb)between the two groups(P<0.05),while there was no significant difference in free thyroxine(FT4),free triiodothyronine(FT3),thyroid stimulating hormone(TSH)and thyroglobulin antibody(TGAb)between the two groups(P>0.05).5.The levels of FBG and triglyceride(TG)in TN group were higher than those in non-TN group,with statistical significance(P<0.05).There were no significant differences in blood routine,ESR,liver function,kidney function,24-hour urinary protein quantification,IgA,IgG,IgM,complement(C3,C4)and other indicators(P>0.05).Conclusion:1.The incidence of SLE with TN can be higher than the normal population;2.The incidence of SLE with TN is age-related and can increase with age,but gender,weight and duration of disease have no significant effect on the incidence of TN.3.The incidence of thyroid dysfunction in patients with SLE and TN is high,and is common in SCH;4.Clinically,SLE patients with high levels of FBG,TG and TPOAb should pay attention to the screening of TN.The rate of missed diagnosis should be reduced and thyroid morphology should be monitored to prevent malignant transformation.
