Background: Colorectal cancer(CRC),the third major cause of mortality among various cancer types in the world.Although medical technology and health awareness are constantly improving,the occurrence of CRC is still widespread in various countries.The treatment of colorectal cancer is still based on surgical resection,supplemented by chemotherapy.However,complications often occur after surgery,and infection is the most common one.Severe postoperative infection can affect the recovery process of patients.In severe cases,even the life of the patient is endangered,the hospitalization time is increased,the patient's pain is increased,and the medical expenses are also increased,which has extremely adverse effects on the patient.Studies have shown that postoperative infection will delay the progression of chemotherapy,and the five-year survival rate and survival time will be reduced.Numerous studies have found that the imbalance of intestinal flora is closely related to the occurrence of colorectal cancer.The intestinal flora plays an important role in maintaining the stability of the internal environment.Probiotics,as part of their health effects,have found that probiotics play an important role in colorectal diseases as their research progresses.Probiotics are a kind of active microorganisms beneficial to the host.They are the general term for active beneficial microorganisms that are colonized in the human intestines and reproductive system and can produce exact health effects,improve the balance of micro-ecology of the host,and exert beneficial effects on the intestinal tract.Probiotics have antibacterial activity and can improve their side effects and enhance the tolerance of treatment when applied simultaneously with antibiotics.In view of the above reasons,scholars have done a lot of research on how to reduce infection after colorectal cancer surgery.There is still controversy about whether preoperative application of probiotics can reduce postoperative infection of colorectal cancer.This article will adopt evidence-based medicine to evaluate the relationship between preoperative probiotics for postoperative infection of colorectal cancer,and provide evidence-based medicine for reducing postoperative infection in patients undergoing colorectal surgery.Objective: To systematically review the effect of preoperative probiotics on postoperative infection of colorectal cancer.Obtain the relationship between probiotics and postoperative infection of colorectal cancer by meta-analysis.Defining the role of probiotics in postoperative infections of colorectal cancer.Provide scientific basis for prevention and control of postoperative colorectal cancer.Methods: A literature search was conducted using Pubmed,Embase,China Biomedical Literature Database(CBM),CNKI,VIP,Wanfang database to collect randomized controlled trials(RCT)of preoperative probiotics and postoperative infections for colorectal cancer at home and abroad.The search period is from January 2000 to September 2018.We evaluated the quality of literature and extracted data from each included study according to the selection criteria.Then we used Rev Man 5.3 provided by the Cochrane Collaboration for analysis.Results: A total of 327 related literatures were retrieved,and finally 6 randomized controlled studies met the requirements and were included in the analysis.A total of 370 subjects(185 were the experimental group and 185 were the control group)were included in the analysis.Meta-analysis showed that preoperative bowel preparation with probiotics reduced postoperative infection rates compared with controls(OR=0.32,95% CI: 0.19 to 0.57).The difference was statistically significant(P < 0.01).Conclusion: Preoperative application of probiotics can reduce the incidence of infection in patients after colorectal cancer surgery.Therefore,we recommend preoperative preparation of probiotics before surgery.
