Purpose:To investigate the correlation between blood pressure variability and prognosis in acute stage of anterior circulation infarction.Method:Prospective analysis of patients admitted to the neurology ward of the first affiliated hospital of Xinjiang medical university due to acute anterior ischemic stroke between January 2018 and July 2018.Patients'basic information on admission,risk factors of cerebrovascular disease,NIHSS score at admission and relevant clinical data were respectively registered.The 24-hour dynamic blood pressure monitor was used to obtain various indicators related to blood pressure variability.According to the modified Rankin scale score at 3 months,patients with anterior circulation infarction were divided into the group with good prognosis?MRS 0-2?and the group with poor prognosis?MRS 3-5?,and the statistical differences between the two groups were compared.Univariate analysis was followed by multivariate logistic regression model to analyze and explore the correlation between blood pressure variability and 90-day prognosis in patients with acute anterior circulation infarction.Result:1.Univariate analysis of patients'basic data showed that compared with the group with a good prognosis,the group with a poor prognosis had more patients with a history of coronary heart disease;2.After univariate analysis of patients between the two groups,24hSBPSD,24hDBPSD,24hDBPcv,daytime SBPmean,daytime SBPSD,daytime SBPcv,daytime DBPSD,daytime DBPSD,daytime DBPcv,and nighttime DBPcv were all statistically significant,while the differences of other indicators between the groups were not statistically significant.3.The results of multivariate regression analysis showed that the history of coronary heart disease and 24hSBPsd were independent factors leading to poor prognosis and independent risk factors.24hSBPsd is also an independent risk factor,indicating that the larger the 24hSBPsd is,the more likely it is to have a poor prognosis.Conclusion:24-hour systolic blood pressure variability is an important predictor of poor prognosis in patients with anterior circulation infarction.
