Objective: To investigate the efficacy of primary hip arthroplasty in the treatment of end-stage brucella hip arthritis.Method: A retrospective analysis of 8 patients(8 hips)with end-stage hip brucella arthritis in our hospital from January 2009 to January 2018 treated with primary total hip arthroplasty combined with anti-inflammation chemotherapy.Patients are all male(8 patients,8 hips);aged 37-70 years,mean age 54.6 years old,all patients had hip pain time of 4 to 48 months,an average of 23.9 months.All patients were followed by Harris hip score to evaluate hip joints function.Results: All patients were followed up for 9 to 92 months,with an average of 23 months.One patient developed fever and night sweat symptoms after 1 year of operation.The patients were treated with oral medication.Symptoms were relieved after 3 months with continuous use of oral drugs,no recurrence was seen at the last follow-up.One patient had a posterior hip dislocation on the third day after surgery and was discharged after manual reduction.The remaining 6patients had no signs of recurrence of brucellosis at the last follow-up.All patients had no signs of lossen prosthesis at the last follow-up.Patients’ Harris hip scores increased from an average of 30.11±5.88 points(24-45 points)(24-45 points)before operation to an average of 90.23±6.65 points(65-96 points)at the last follow-up.Conclusion: Primary total hip arthroplasty in patients with end-stage brucella hip arthritis is safe and feasible,which can achieve the purpose of controlling infection,relieving pain and improving hip function.
