ObjectiveTo investigate the clinical effect of total hip arthroplasty(THA)on patients with rheumatoid arthritis(RA)through collection of the clinical and follow-up data from January 2014 to November 2016 in Linyi Peoplel Hospital. The evaluation included the follow-up before operation,1 month,3 months after operation and 6 months after operation,as well as the Harris score of THA and the corresponding points. At last,we carried out statistical analysis with SPSS software with the hope of development towards the treatment of emergency problems and the efficacy of surgery.Materials and MethodsA total of 12 patients with RA,including 5 males and 7 females, aged 45-72 years, with an average age of 51.7 years old, underwent THA from January 2014 to November 2016 in our hospital. Most patients with both the left and right sides of rheumatoid hip arthritis, asked for THA of the side with joint pain, deformity and dysfunction. Two cases(2 hips)of female patients with acetabular retraction, according to Sotelo-Garza and Charnley classification, the distance of acetabular point over Kohler's line:type ?(acetabular retraction to 4mm),type ? Acetabular retraction to 13mm),no acetabular retraction greater than 15mm.Patients were always with varying degrees of persistent hip pain, hip contracture deformity, joint dysfunction pre-operative.The pre-operative Harris score was 29.43 ± 5.59 points. All patients were treated with biologic hip prosthesis, using conventional posterior lateral approach of hip, routine functional exercise post-operative and telephone follow-up to obtain the Harris score of THA and the corresponding points before operation,1 month,3 months after operation and 6 months after operation, following with the statistical analysis.ResultTwelve patients were all followed up the Harris score of THA and the corresponding points before operation,1 month,3 months after and 6 months after operation. The results showed that all patients with post-operative hip and lower limb pain symptoms were significantly relieved. The limbs of all patients were normal under X-ray after 1 month,3 months and 6 months. All patients underwent operation were I/A healing, except 2 cases delayed healing and dismantling stitches to 14 days due to diabetic. All patients were out of hip infection, deep vein thrombosis, lung Infection, urinary tract infection and pressure sores. The Harris score of 1 month post-operation was 76.94 ± 4.18 points,3 months of 85.96 ± 5.8 points,6 months of 90.59± 5.09 points.X-ray showed that all patients had normal prosthesis position,no shift,no loosening and other signs.Quality of life post-operative was significantly improved and Harris score was significantly improved after total hip arthroplasty(P<0.005).ConclusionsThere are better effects of THA on treatment of rheumatoid hip arthritis, which can significantly improve the symptoms of pain, functional activities, hip mobility range and patients get good gait after surgery. Therefore, artificial total hip arthroplasty on rheumatoid hip arthroplasty patients with joint function reconstruction, the current restoration of hip function,to relieve joint pain and improve the life quality of patients.THA is the ending treatment for the restoration of hip function.
