ObjectiveTo investigate the situation of transitional care demands of elderly patients with hypertension,and analyse the main influencing factors of transitional care demands of elderly patients with hypertension.According to the influencing factors,to help the nursing staff implement personalized care service measures for the patients with different demands,and provide the basis to improve the life quality of elderly patients with hypertension.At the same time,it also laid a theoretical foundation for the further construction of transitional care model for elderly patients with hypertension.MethodTo select the elderly patients with hypertension who were in the tertiary-grade class-A hospital in yunnan Province from January 2018 to September 2018,and use self-designed questionnaire to conduct the survey of transitional care demands.To evaluate reliability and validity of the scale.The data are analyzed by using SPSS21.0 software.The basic data and demand rate were statistically described by frequency and composition ratio.Data of normal distribution are described by using().Independent sample t test,single factor analysis of variance,LSD test,Pearson or Spearman correlation analysis and multiple linear stepwise regression analysis were used to predict the relevant factors affecting the demand for transitional care.Results1.In this study,the age of elderly patients with hypertension was mainly 65 to 75 years sx±old,and the educational level was mainly below junior high school.The main source of income for most patients was retirement pension,and most of them have a monthly income of1500-3000 yuan.65.08% of the patients did not receive health education after discharge,and51.2% of the patients had a course of disease of more than 10 years.The self-efficacy(26.83±5.20)and quality of life(60.26±10.15)of the elderly patients with hypertension were at the middle level.2.After discharge,71.86% of elderly patients with hypertension wished to receive transitional care services.The total score of demand for transitional care services was 3.67 ±0.41.The contents of transitional care with high demand were knowledge related to complications and prevention guidance,drug knowledge and safe drug use guidance,and guidance based on syndrome differentiation.The scores were 4.26±0.54,4.21±0.60,4.18±0.67 respectively.Transitional care services modes with high demand of patients were telephone follow-up,health instruction manual and outpatient review,accounting for 91.51%,82.55% and 80.19% respectively;the most demanding way for transitional care services providers was multidisciplinary transitional care group,accounting for 51.42%;as far as the duration of transitional care services was concerned,the patients who chose more than one year were the most,accounting for 45.28%;as far as the frequency of follow-up was concerned,55.66% of the patients chose once a month.3.Through the analysis of multi-factors influencing the total demand of transitional care services and the demand of various dimensions for elderly patients with hypertension,it was found that the main factors influencing the demand of transitional care for elderly patients with hypertension were education level,awareness of complications,course of disease,community hospital visits,willingness to transitional care,self-efficacy and quality of life.Conclusion1.The score of the demand for transitional care services of elderly patients with hypertension was on the upper middle level.Generally speaking,the patients have a strong desire to carry out transitional care services in hospitals.2.Elderly patients with hypertension hope to obtain long-term transitional care services through more convenient ways.Medical staff can implement transitional care for patientsthrough telephone follow-up,health guidance manual,outpatient review and other ways.Especially,they should focus on transitional care projects with high demand,mainly including knowledge of complications and prevention guidance,drug knowledge and safe drug use guidance,and guidance based on syndrome differentiation.3.Elderly patients with hypertension with high educational level,knowledge of hypertension complications,short course of disease,frequent visits to community hospitals,strong willingness to transitional care,high self-efficacy and poor quality of life have a higher demand for transitional care.Medical workers should focus on meeting the needs of this part of the population.
