Purpose: Excessive Lateral Pressure Syndrome is a very common type of knee joint injury.Pain distresses patients and affects their daily life,such as walking up and down stairs.This study used muscle strength training and stretching relaxation to intervene in Excessive Lateral Pressure Syndrome,and compared the effects of the two methods,compared the advantages of the two methods,the basis of clinical treatment is provided for this disease.Methods: 41 patients with Excessive Lateral Pressure Syndrome in Nanjing were recruited by orthopaedics department of provincial Chinese medicine hospital.Before and after the experiment,standard knee pain questionnaires were used to investigate the related manifestations of knee pain.Neuromuscular reaction time test system was used to evaluate the starting time of medial and lateral oblique femoral muscles and the difference between them.Dynamic hardness and other muscle states.According to the pathogenesis of the patellofemoral lateral hypertension syndrome,two types of exercise rehabilitation methods were established: muscle strength training(including medial oblique muscle stimulation of the vastus medialis,gluteus medius,gluteus maximus training)and stretching relaxation training(including active and passive stretching combined with foam shaft relaxing lateral femoral muscle and iliofemoral tract),comparative analysis of the effects of the two exercise rehabilitation methods for the Excessive Lateral Pressure Syndrome.Results: Of 41 subjects,61% were female;only 44% had exercise habits;41.4% were between 30 and 40 years old.Muscle strength training increased the total score of Kujala knee joint scale(P < 0.05),while stretching group significantly decreased the score of VAS subjective pain scale(P<0.05).There was no significant difference in starting time between VMO and VL between strength group and stretching group.The stretching group slowed down the starting time of VMO when the subjects were in the semisquatting knee dominant position,semi-squatting autonomous position,sitting leg flexion and extension 0-load position(P <0.05).The stretching group trained the VMO in the healthy side to do semi-squatting knee dominant position,semisquatting hip dominant position and sitting leg flexion(P<0.5)The start-up time of 15 kg stretching was slower than that before the experiment(P<0.05);the start-up time of the VL muscle of the healthy and sick side in the stretching group was slower than that before the experiment(P<0.05).Muscle strength training significantly reduced the muscle tension of the affected medial femoral muscle and the muscle elasticity of the affected medial gluteal muscle(P<0.05);stretch group training reduced the dynamic hardness of the affected lateral femoral muscle and the healthy medial femoral muscle(P<0.05);stretch group training prolonged the Deborah number of the affected medial gluteal muscle and the recovery time after deformation(P<0.05);stretch group training prolonged the VMO muscle of the affected side(P<0.05);stretch group training made the medial oblique FEM The IEMG of the medial oblique femoris muscle of the healthy side decreased significantly in the upper step and the sitting position leg flexion and extension isometric movements(P<0.05);the IEMG of the VMO muscle of the healthy side decreased significantly in the sitting position leg flexion and extension isometric movements(P<0.05);the IEMG of the lateral oblique femoris muscle of the healthy side decreased significantly in the sitting position leg flexion and extension isometric movements(P<0.05);and the IEMG of the VL muscle of the healthy side decreased significantly in the sitting position leg flexion and Obvious.Conclusions: 1.Among the patients of Excessive Lateral Pressure Syndrome,females were more than males;less than the half of subjects had exercise habits;the subjects aged 30-40 years were the most.2.Muscle strength training plays a positive role in improving the comprehensive performance of knee joint in daily life of Excessive Lateral Pressure Syndrome;stretch training has a significant effect on relieving knee joint pain caused by Excessive Lateral Pressure Syndrome.3.The difference of the starting time between VMO and VL muscles is not the decisive factor of the Excessive Lateral Pressure Syndrome.The relaxation time of stretching combined with foam shaft relaxation makes the starting time of the vastus lateralis and lateral oblique muscles slower than before.4.The effect of muscle strength and stretching training on the muscle tension of leg muscles under relaxation is not very obvious,but the effect of stretching on the dynamic hardness of some muscles is obvious.5.The EMG integral values of the VMO and VL tended to decrease in the closed-chain test,presumably because the gluteal muscles participated more,while the EMG integral values of the VMO and VL tended to increase in the open-chain test,and the EMG integral values of the medial and lateral oblique femoris tended to decrease more obviously by stretching.
