Objective:To investigate the effect of high uric acid level on cognitive function in bipolar ? patients;Is there a correlation between impulsivity and cognitive function in bipolar ? patients.Method: Randomly collected in December 2017-May 2018 hospitalized in a hospital,voluntary informed consent to participate in this study and signed the 67 cases of bipolar ? manic patients(research group)and 40 cases of age,gender,education of matching of healthy people(control group)health,according to the treatment of high blood uric acid and gout China expert consensus,according to the serum uric acid level to bipolar ? manic group is divided into high uric acid hematic disease group(> 420umol/L,men women > 360 umol/L)and blood uric acid level in normal group.The scale included: self-designed general situation questionnaire,young's manic scale(YMRS),Barratt impulsivity scale(BIS-11),and schizophrenia cognitive function battery--consensus version(MCCB).Serum uric acid levels in the two groups were measured,manic symptoms and severity in the study group were assessed using the young's manic scale(YMRS),impulsivity levels in the two groups were assessed using the Barratt impulsivity scale(BIS-11),and cognitive function in the two groups was assessed using the schizophrenia cognitive function battery(MCCB).Results:1.Team in MCCB total score and the information processing speed,reasoning and problem solving,learning,working memory,visual words,pay attention to/alertness scale scores were significantly higher than that of healthy control group,the difference was statistically significant(P < 0.01).2.The team serum uric acid level is significantly higher than healthy control group,difference was statistically significant(t = 2.90,P <0.01).3.The scores of motor impulsivity,cognitive impulsivity and unplanned impulsivity in the study group were significantly higher than those in the healthy control group,with statistically significant differences(Z=-4.00,-6.11,-2.08,P < 0.05 or P<0.01).4.The score of reasoning and problem solving cognition in the group with high uric acid level in bipolar ? manic episode was significantly lower than that in the group with normal uric acid level,and the difference was statistically significant(Z=-2.507,P< 0.05).5.In the bipolar ? group,there was a significant negative correlation between cognitive impulsivity and word learning,visual learning,attention/alertness,and total MCCB score(r=-0.247,-0.269,-0.275,-0.244,P<0.05).The scores of motion impulsivity factor,unplanned impulsivity and total impulsivity were significantly positively correlated with the information processing speed(r=0.297,0.388,0.272,P<0.01).Conclusion:Patients with bipolar ? mania have significant neurocognitive impairment,accompanied by increased serum uric acid levels and impulsive levels.Bipolar I manic episode patients with hyperuricemia had significantly impaired executive function compared with bipolar ? manic episode patients with normal uric acid levels.The higher the cognitive impulsivity(the impulse to pay attention),the worse the verbal learning,visual learning,attention/alertness,and cognitive function.
