Objective To understand the current epidemic status of hepatitis B virus(HBV)infection and serological immunization against hepatitis B among people aged between1 and 59 in Ma'anshan city,and evaluate the prevention and control effect of hepatitis B after the adjustment of hepatitis B vaccination strategies,and furthermore,to assess the knowledge related to the disease of hepatitis B and health and disease prevention awareness among the subjects aged over 15 years old.Methods Total of 3460 people aged from 1 to 59 years old were selected with multistage sampling method from 10 investigated sites in the 6 counties(districts)of Maanshan city.Questionnaires collected from each of the individual in a separate room.Questions included demographic characteristics,history of hepatitis infection and hepatitis B vaccination,awareness of hepatitis B related knowledge and method of hepatitis B control and prevention.In addition,3-4ml venous blood were collected to detect HBsAg,HBsAb,HBeAg,HBeAb and HBcAb by domestic enzyme linked immunosorbent assay(ELISA)reagent.EpiData 3.1 software was used to establish a database and all of the information collected from participants,after double entry and validation.SPSS 16.0 were used for data analysis,Qualitative data were described by rate,proportion and ratio.Chi-square test and trend test were used to examine the relationship between groups.Multivariate analysis by non-conditional Logistic regression analysis was used to analyze the difference of rate on knowledge related to Hepatitis B virus infection,A P value of<0.05 was considered statistically significant.Results The total positive rate of HBsAg,HBsAb and HBV infection among people aged between 1 and 59 in Ma'anshan city was 3.32%,51.21%and 29.16%respectively.HBsAg positive rate was extremely low(0.18%)and HBV infection rate was 3.57%in the age group under 25 years old,The HBV infection rate increased with age in the age group over 25 years old(Trend test?~2=62.591,P<0.001).The average positive rate of HBsAb among people aged 5 to 19 years old was only 40.77%.Vaccination rate of hepatitis B vaccine was 55.95%.Vaccination rate decreased with age(Trend test?~2=1984.342,P<0.001),the rate reached 100%in people aged<15 years.The differences of HBsAg,HBsAb positive rate and HBV infection rate was all statistical significant(P<0.01)among different immunological histories,The positive rate of HBsAb was significant higher after the adjustment of antigen levels(?~2=90.331,P?0.001).Vaccination rate of hepatitis B vaccine was highest in medical workers than others,the difference was statistically significant(all P<0.05).High HBV infection rate(54.06%)and low hepatitis B vaccination rate(12.99%)in farmers.People who lives in urban area,medical and nursing staff,highly income,highly educated and has the history of hepatitis B vaccination have a high rate of awareness of knowledge about hepatitis B,and the difference was statistically significant(P<0.001).The HBV infection rate was lower(?~2=12.404,P?0.001)and the hepatitis B vaccination rate was higher(?~2=96.538,P?0.001)among those with related knowledge of hepatitis B.Conclusion The strategy of hepatitis B vaccination conducted in Ma'anshan city has achieved good results in the prevention and control of hepatitis B.HBsAg positive rate and HBV infection rate were further decreased in people aged<25 years.In 2012,Anhui province implemented the measures to adjust the antigen content of hepatitis B vaccine,which effectively improved the positive conversion rate of HBsAb in the inoculated children.In this survey,people over 25 years old have low HepB inoculation rate and high HBV infection rate.The positive rate of HBsAb is low for students between 5 and 19 years old.The rural population has low awareness of hepatitis B knowledge and weak awareness of disease prevention and the low rate of hepb-related knowledge in the population,all these will likely to become the emphases attention problem.
