Objective:To understand the infection status and epidemic trend of hepatitis B virusin different parts and different populations of the Hebei province, and toanalyze the infection factors of hepatitis B virus in the province; to understandthe hepatitis B vaccination rate of people who were under19years old in thedifferent areas of the province,and analyze its relationship with viral infectionrate.Method:Seroepidemiological studies was carried out in the resident population inHebei area, using random sampling method of multistage cluster system in2011.Twenty-two counties were randomly selected. According to thesystematic sampling, the residents are extracted within the family as a unit atthe age of1~59, and carries on the questionnaire survey, and at the same timeextract the vein blood5ml. Using enzyme-linked immunosorbent assay(ELISA) detects hepatitis B virus surface antigen (HBsAg), hepatitis Bsurface antibody (anti-HBs) and hepatitis B virus core antibody (anti-HBc)in blood. Questionnaires and test results were input into Epi data3.10software,and using Excel and Epi info software for data collection and analysis.Results:1In this survey, we pick22towns (districts) of11cities, and the totalsample is5072, of which4567valid samples with epidemiological data andlaboratory test results. So the response rate is90.04%. Investigation number ofcity population is2311, and the number of rural population is2256,accounting for50.60%and49.40%of the total population of surveyrespectively. The rate of male and female is1:1.06.2Throughout the population aged1~59of this province, the rate of anti -HBsAg,, anti-HBs, and anti-HBc are3.31%,43.35%and29.67%. Thehighest positive rate of HBsAg is in Langfang (4.99%), Tangshan (1.27%)is the lowest; the highest anti-HBs positive rate is56%in Shijiazhuang.3There was significant difference of HBsAg positive rate of populationin urban and rural areas; the anti-HBs positive rate of city crowd is higherthan in rural areas, the difference was statistically significant; the anti-HBcpositive rate of rural population was higher than in the city, there is astatistically significant difference.4Throughout the investigation population aged1~4, the Vaccination ofhepatitis B vaccine rate and the timely first-dose vaccination rate are94.05%and94.05%, while among people aged5~19, the Vaccination of hepatitis Bvaccine rate and the timely first-dose vaccination rate are70.90%and55.43%.It means that the older the person is, the lower of the full course vaccinationrate and the timely first-dose vaccination rate are.5By using the Logistic regression model to analyze the Relationshipbetween14factors and HBsAg positive, it is found that a HBsAg positivemembers of the family is a risk factor for Hepatitis b infection.Conclusion:1The survey shows that positive rate of HBsAg is3.31%in hebeiprovince, compared with that of9.75%in1992. Positive rate of HBsAgdecreases obviously. According to classification standard of the world healthorganization, Hebei is at the middle level(8%> HBsAg positive rate≥2%).2HBsAg positive rate in the whole crowd approaching7%or less of thecontrol objectives,which was from the national hepatitis B preventionplanning from2006to2010, had been achieved in Hebei province.3HBsAg positive rate in rural areas is higher than the city’s in HebeiProvince, the highest positive rate is among15~29year old crowd,dominated by men.4HBsAg positive rate was negatively correlated with educational level;HBsAg positive rate was not the same in the different nationalities.5Hepatitis b vaccination rate has increased significantly. The whole course vaccination rate and the timely first dose rate in rural and urban hasbeen no difference among the low age group.6By using the Logistic regression model to investigate a variety ofinfluencing factors, it is found that HBsAg positive members of the family is arisk factor for hepatitis b infection.7The anti-HBs positive rate levels of the population are much higherthan that in1992, which means that hepatitis b vaccination has madesignificant effect.
