The Curative Effect Observations Of Two Windows Ilioinguinal Approach In The Treatment Of Pelvis And Acetabular Fractures

Objective:To introduce two windows ilioinguinal approach in treatment of pelvis and acetabular fractures to provide clinical doctors with a more minimally invasive and safer surgical approach.Methods:To make a retrospective analysis on the clinical data during October 2015 to March 2017,of 56 cases of my patients who were treated pelvis and acetabular fracture with two windows ilioinguinal approach and ilioinguinal approach in our hospital and several others by the same team,which were divided into observation group(n=36)and control group(n=20).The observation group used two windows ilioinguinal approach to operate and the control group ilioinguinal approach.The average operation time,the average blood loss,the anatomical reset satisfaction rate and the excellent clinical effect rate were recorded to observe the curative effect.Results:The excellence rate in the observation group is 77.8%,and that in the control group is 80.0%,no statistically significant difference(P > 0.05),The average operation time in the observation group takes 124.48±20.32 min,and the intra-op average blood loss is 220.97±50.76 ml,which is significantly lower than the control group(192.33± 34.25 min,510.45±87.63ml),There were significant difference in the therapeutic results between the two groups(P<0.05).In the observation group,16 cases were anatomical reduction,14 cases were reset satisfaction,6 cases were poor reset,and the satisfaction rate was 83.3%.In the control group,10 cases were anatomical reduction,7 cases were reset satisfaction,3 cases were poor reset,and the satisfaction rate was 85.0%,no statistically significant difference(P>0.05).With regard to the excellent clinical effect rate,in the observation group,22 cases got excellent result,6 cases good,8 cases poor,and the excellence rate was 77.8%.In the control group,13 cases got excellent result,3 cases good,4 cases poor,and the excellence rate was 80.0%,no statistically significant difference(P>0.05).1 case ofdeep venous thrombosis was found in the control group,and the incidence rate was5.0%,no similar case was found in the observation group.2 cases of lateral femoral cutaneous nerve injury were found in the observation group,and the incidence rate was 5.6%,and no similar case was found in the control group.Conclution:The two windows ilioinguinal approach possess benefits,such as minor trauma,shorter time,and less blood loss,which makes this surgical approach a good option for the treatment of pelvis and acetabular fractures.
