Objective To compare the effectiveness between modified-stoppa approach andilioinguinal approach in the treatment of acetabular fracture.Methods The clinical data were retrospectively analyzed, from34patients acetabularfracture treated between February2008to June2012.Of34patients,18patientsunderwent ilioinguinal approach or unite Kocher-Langenbeck(K-L) approachreduction of fracture and combined with fixation (conventional group), and16patients underwent modified-stoppa approach or unite K-L approach combined withfixation (research group). There was no significant diference in gender, age, injurycause, Letournel classification,comorbidities, disease durations between2groups (P>0.05).Results The operation was successfully completed in all patients.The operation time,blood loss, operative complications in the research group were significantly less thanthose in the conventional group (P <0.05). Matta scores were significantly better thanpreoperative scores in2groups.The research group is Matta score higher than theconventional group but there was no significant diference in gender (P>0.05).Research group Merled’Aubigne Postel scores at3,6,and12months were betterthan conventional group but there was no significant diference in gender(P>0.05)Conclusion Compared with ilioinguinal approach,modified-stoppa approach has theadvantages of sufficient exposure that is helpful for fracture reduction and fixation,improve the quality of surgery in treating acetabulum fracture. At the same time canreduce surgical damage and complications. Operating treatment of modified-stoppaapproach combined with early rehabilitation after surgery can recover hip jointfunction as quickly as possible.
