Purpose:To evaluate the clinical efficacy and safety of Targeting-intratumoral-lactic acidosis transcatheter arterial chemoembolization(TILA-TACE)combined with other comprehensive treatment methods(external radiotherapy,125I radioactive particle implantation and percutaneous ethanol injection,etc.)in treatment of huge hepatocellular carcinoma with maximum diameter ?10cm.Materials and Methods:The clinical data of 44 huge hepatocellular carcinoma(the maximum diameter was?10 CM)cases that were treated with TILA-TACE combined with comprehensive treatment methods in the second affiliated Hospital of Zhejiang University Medical College between July 2012 and April 2017 was retrospective analyzed.12 cases were BCLC stage B,32 cases were BCLC stage C.The local therapeutic efficacy was evaluated by contrast-enhanced MRI using the mRECIST criterion.Total survival rate,median survival time,1 year-,2 years-and 3 years cumulative survival rate were also followed up.Results(1)44 patients received 153 courses of TILA-TACE treatment totally,among these patients,26 patients received comprehensive treatment,the technical success rate was 100%.(2)Among 44 cases,CR was achieved in 19 cases(43.2%),PR was achieved in 20 cases(45.5%),SD was achieved in 3 cases(6.8%),while PD was seen in 2 cases(4.5%),the objectively effective rate was 88.6%and disease control rate was 95.5%.(3)The total survival time between 2 and 60 months,the median survival time was 18.1 months,and the cumulative survival of 1 year-,2 years-and 3 years-was 59.1%?18.2%and 9.1%respectively.(4)The main adverse events were fever,pain,vomiting and liver function damage.No treatment-related death or other serious complications were found.Conclusions:The local therapeutic efficacy,survival benefit and security of TILA-TACE combined with comprehensive treatment methods in the treatment of huge hepatocellular carcinoma(?10cm)was satisfied and this new therapeutic strategy worthy of further investigation. |
