Retrospective Analysis Of The Status And Influencing Factors Of Frailty In Elderly Fracture Patients

Objective:This research aims to analyze the current status and relationship of pre-fracture frail,comorbidity index,self-care ability and nutritional risk in elderly patients,and to explore the influencing factors of pre-fracture frail in elderly patients,so as to provide a theoretical basis for delaying the debilitation of the elderly and preventing the occurrence of fractures after falls.Methods:This study is a descriptive study in non-experimental studies.A convenient sampling method was used to conduct a questionnaire survey on elderly fracture patients who met the inclusion criteria in six comprehensive hospitals in Jilin province.A total of 270 questionnaires were distributed in this study,and 262 valid questionnaires were returned.The effective recovery rate was 97%.The questionnaire is divided into six parts,the patient social demographic data,the frail scale,the comorbidity index quantity scale,the self-care ability scale,the instrumental self-care ability scale,and the nutrition risk scale.SPSS 19.0 statistical software was used for statistical analysis and statistical methods such as percentage,mean,standard deviation,independent sample t test,one-way ANOVA,and chi-square test were used.The variables of significance were included in multiple linear regression analysis to screen out the influencing factors of senile debilitation.Results:(1)The total score of frailty pre-fractures in elderly fracture patients was 0-5,with an average of(1.55±1.21)points,of which 69 were robust,accounting for 26.3%,119 were pre-frail,accounting for 45.4%,and 74 were frail,accounting for 28.2%.The age,ethnicity,education level,marriage,residence mode,economic status,medication status,fracture site,and history of falls in the Past year were significantly different in the total score of frail elderly(P<0.05).(2)The total score of CCI in elderly fracture patients was(4.44±1.27).The score of 0-2was 1.9%,the score of 3-4 was 58.8%,and the score of ?5 was 39.3%.The age,ethnicity,education level,marriage and medication status of elderly fracture patients were statistically significant(P<0.05).(3)The total score of body function self-care ability in elderly fracture patients was(80.57±27.35),among which heavy dependence accounted for 11.8%,moderate dependence accounted for 7.6%,mild dependence accounted for 38.2%,and independent self-care accounted for 42.4%.The age,education level,marital status,living style,medication status,and fracture site scores of the elderly patients with fractures were statistically significant(P<0.05).(4)The scores of instrumental self-care ability in elderly fracture patients(15.74±8.19),and those with functional impairment accounted for 64.5%.The age,education level,marital status,residence pattern,medication status,and fracture site of the elderly fracture patients were statistically significant(P<0.05).(5)The total nutritional score of elderly fracture patients(11.47±1.98),52.3%of those with good nutritional status,43.1%of people with malnutrition risk,and 7.6%of malnourished people.The age of elderly fracture patients were statistically significant(P<0.05).(6)There was a correlation between frailty,comorbidity index,self-care ability and nutrition in elderly fracture patients.The comorbidity index was positively correlated with frailty(r=0.430,P<0.01);life self-care abilities were negatively correlated with frailty(r=-0.685,P<0.01);instrumental self-care abilities were positively correlated with frailty(r=0.619,P<0.01);nutrition risks were negatively correlated with frailty(r=-0.287,P<0.01).(7)Multivariate analysis showed that ethnicity,fracture site,ethnicity,history of falls in the past year,medication status,comorbidity index,nutrition,instrumental self-care ability,and the age were the influencing factors of frailty in elderly fracture Patients.Conclusions:(1)The incidence of pre-fracture frail in elderly patients is 28.2%higher than that in the community;(2)There is a correlation between frail,comorbidity index,self-care ability and nutritional risk in pre-fracture elderly patients;(3)Multivariate analysis showed that the influencing factors related to the total score of senile frailty were the fracture site,ethnicity,history of falls in the Past year,medication status,comorbidity index,nutrition and instrumental self-care ability.
