| Objectives:To compare the efficacy,safety and cost-effectiveness between endoscopic submucosal dissection and surgery for the treatment of early gastric cancer and precancerous lesions.Methods:A total of 78 early gastric cancer or precancerous patients who had been admitted to the Handan center hospital during September 2016 to December 2018 were included in this study.Patients were divided into ESD group and surgical group according to treatment methods.Collected the clinical data of patients,including gender,age,lesion site,lesion size,pathological diagnosis,follow-up period,infiltration depth,incidence of complications,en bloc resection rate,complete resection rate,curative resection rate,survival rate,recurrence rate,time of hospitalization,total treatment cost and operative time.To compare the efficacy,safety and cost-effectiveness between two groups.Results:46 underwent ESD and 32 underwent surgery.The incidence of complications in ESD group was 6.5%(3/46).The complete resection rate and curative resection rate of ESD group was 95.7%(44/46).The average operation time of ESD group was 91.44±23.91 min.The average time of hospitalization in ESD group was14.45±2.60 days.The average total treatment cost of ESD group was24322.87±3671.86 yuan.The incidence of complications in surgery group was25.0%(8/32).The complete resection rate and curative resection rate of surgery group was 100%.The average operation time of surgery group was 189.17±33.44 min.The average time of hospitalization in surgery group was 22.07±4.76 days.The average total treatment cost of surgery group was 59359.12±18152.67 yuan.There was no patient dead.The recurrence rate of two groups was 0%.There was no significant difference in complete resection rate and curative resection rate between two groups(p>0.05).The incidence of complications,operative time,hospital stay time and total treatment cost were significantly less in the ESD group than the surgery group(p<0.05).Conclusions:ESD is safe and feasible in the treatment of early gastric cancer and precancerous lesions with the advantages of less trauma,faster recovery and less complications.Its clinical efficacy is similar to surgery. |
