Background:Traditional esophagusradicalsurgical is the main tre atment of early esophageal cancer.In recent years,endoscopic submu cosal dissection(ESD)as a new treatment method is maturegradually,but comparision of curative effectsbetween ESD and esophageal radical surgery are few.Objective:The purpose of this study is to evaluate the efficacy of ESDand radical surgery for esophageal cancer and precancerous lesions.Method:30patients diagonosed as early esophageal cancer and precancerous lesions during and received ESD during November 2015 and January 2018 in Affiliated Hospital of Jining Medical College were collected and divided into ESD group.At the same time 33 cases,who received esophageal radical surgery were divided into Surgery group as control group.Weanalyzed and compared their path ological characteristics(e.g.,age,gender,pathological diagnosis),relevant expenses(operation,examination,hospitalization),and the resul ts(operation time,hospitalization days,complications,follow-up,etc.)by retrospective study.Result:There were no significant differences in age,gender,pat hological diagnosis and follow-up time(P > 0.05);the lesion resection rate in different level of the whole block resection rate,complete resection rate and curative resection rate had no significant differ ences(P>0.05);The related expenses(surgery costs,inspection fee,hospitalization expenses)of Surgery group was higher and it was significantly different(P < 0.05);The operation time and hospital days in surgery group were longer,and there were significant differenc e(P < 0.05);there were significantly differet in intraoperative postoperative complications(P < 0.05);the incidence of complication wa s lower in the EDS group;There was no significant differences in overall survival(P > 0.05);overall survival rate was 93.94%(31/33).During the follow-up,patients reviewed the gastrointestinal symptoms(such as nausea,vomiting,acid reflux,heartburn,bloating,loss of appetite,weight loss,etc.)and endoscopic performance,and the ESD group was superior to surgery group.Conclusion:There were no difference in overall survival and lesion resection between ESD group and surgery group.The curative effect of two groups was similar.The ESD group had lower incidence of complication and costs compared with surgery group,and patients had small trauma,enjoyed high quality of life.
