A Study On The Current Situation And Influencing Factors Of Activities Of Daily Living Of The Elderly In Nursing Homes In Urumqi

Objective: This study aimed to investigate the current situation of the activities of daily living(ADL)and to explore its influencing factors of the elderly in nursing homes in Urumqi.It offers the direction for the government to formulate the policies of pension-related and the scientific basis for the early prevention and intervention strategy of the elderly's daily life capacity disorder.Methods: In this study,stratified random cluster sampling was adopted to select 8 nursing homes in three districts of Urumqi,and761 elderly people in nursing homes were surveyed by using the General Data Questionnaire,the Barthel Index Rating Scale,the Geriatric Depression Scale-15(GDS-15)and Social Support Rating Scale(SSRS).The Barthel Index Rating Scale was used to evaluate ADL of the elderly,and the other three scales were used to collect possible influencing factors affecting ADL of the elderly.Results: 1.The average score of Barthel index of 761 elderly people in nursing homes in Urumqi was 70.12±23.52,The prevalence rate of disability for ADL in the elderly was 70.4%,238(31.3%)people suffering from mild dysfunction,179(23.5%)people with moderate dysfunction,and119(15.6%)people had a severe dysfunction.2.The prevalence of chronic diseases was high,84.1% of the elderly with chronic diseases in this study,32.6% elderly suffered from a chronic disease,23.4% of elderly suffered from two kinds of chronic diseases;Those suffered from three or more chronic diseases was 28.1%;The study also found that the prevalence of hypertension was the highest in the elderly in nursing homes,reached 45.3%,the prevalence of diabetes was 32.5%,the incidence of coronary heart disease was 27.3%.3.The average score of the elderly depression was 4.83±3.44,and the depression detection rate was 56.6%,of which 363 had mild depression,accounting for 47.7%,68 people suffering from moderate and severe depression,accounting for8.9%.4.The results of Univariate analysis of ADL of the elderly in nursing homes showed that: the elderly with different region,gender,age,marital status,cultural level,self-evaluation economic status,self-rated labor intensity,smoking,drinking,sports,hobbies,number of chronic diseases,depression and social support,difference was statistically significant(P<0.05).5.Multivariate logistic regression analysis of ADL of the elderly in nursing homes indicated that age,marital status,cultural level,self-evaluation economic status,self-rated labor intensity,sports,number of chronic diseases,depression and social support may be factors influencing the activities of daily living.Conclusion: The overall level of ADL of the elderly in nursing homes in Urumqi is not high.suggesting that the managers of nursing homes should pay attention to the elderly,widowed,financially poor,coexistence of multiple diseases,depression and low level of social support,actively carry out prevention and treatment of chronic diseases,focus on high-risk groups to intervene,advocate healthy lifestyles and strengthen social support for the elderly in nursing homes.
