Objective To study the changes of serum cystatin(CysC)and homocysteine(Hcy)in patients with acute cerebral infarction(ACI).Methods 120 ACI patients who were diagnosed and treated in the outpatient and the ward of liaocheng TCM hospital from March 2014 to March 2016 were selected as the patient group,including 72 males and 48 females,aged 45 to 78 years old,and average age(64.9 ± 9.8),which all met the diagnostic criteria of the 2014 guidelines for the diagnosis and treatment of acute ischemic stroke[1]MRI or/and MSCT scan of the brain were performed in all patients.All the patients were in the first onset,and the time range from onset to hospital for treatment was from 0.5h to 72h,and the neurological defect was in stable condition.All the patients were excluded from the following diseases that were affecting CysC and Hcy metabolism:diabetes,cerebral hemorrhage,thyroid disease,tumor,autoimmune disease,severe heart disease,liver and renal insufficiency,anemia,haemophilia,gout,epilepsy,scleroderma,gastric disease,aortitis,etc.No special treatment history(radiotherapy,chemotherapy,etc.)were recorded for nearly 1 month;The drugs affecting CysC and Hey metabolism,such as various immunosuppressive agents,estrogen,folic acid,vitamin B12(VitB12),vitamin B6(VitB6).antiepileptic drugs,methotrexate,theophylline,were not taken in the past 3 months.The control group included 120 healthy people,70 males and 50 females,aged from 44 to 79 years old,with the average age(63.6± 9.8).The serum contents of CysC and Hey in the patient group were measured in the second day after admission,and that in the outpatients were measured in the same day as the patients came to outpatient department.The serum contents of CysC and Hcy in the control group were conducted in the same day in the examination center as they came for physical examination.The serum CysC and Hey contents of 120 ACI and 120 healthy subjects were measured by colloid gold granule immunoturbidimetry and enzyme circulation.Statistical analysis was performed using SPSS 17.0 softwareResults The results of this study showed that the serum CysC(1.16±0.21)mg·L-1 and Hcy(15.83±3.15)umol·L-1 inthe ACI group were higher than those CysC(0.97±0.18)mg·L-1and Hcy(13.55±2.22)umol·L-1 in the healthy control group.There were significant differences between the two groups(P<0.01).CysC and Hey levels also increased with age.CysC(1.26 ±0.11)mg·L-1 andHcy(16.89±2.50)umol·L-1 in patients older than 60 years were both higher than those CysC(0.97±0.28)mg·L-1 and Hcy(13.99±+3.32)umol·L-1 in patients younger than 60 years in the patient group.There were significant differences between the two groups(P<0.01)The level of serum CysC and Hcy in male was slightly higher than that in female(P>0.05),and the difference was not statistically significant.Serum CysC and Hcy levels were significantly positively correlated with ACI,respectively(r=0.670?0.874,P<0.01),and there was a significant correlation between serum CysC and Hcy(r=0.721,P<0.01)in ACI.Conclusion The results showed that the serum CysC and Hey levels in ACI groupwere significantly higher than those of the control group,P<0.01,with statistically significant differences.TheHCysC and Hhcy detection rate in the ACI increased significantly compared with that in healthy control group.With age increase,the serum CysC and Hey levels also increased,the serum CysC and Hey levels in patients with age>60 years were higher than that in patients with age 60 years or less.The level of CysC and Hey is significantly positively correlated with the incidence of ACI.The CysC an Hey synergitically participate in the incidence of ACI.It has been fully confirmed that HCysC and Hhcy are closely related to the occurrence and development of ACI.Detection and control of serum CysC and Hey levels can effectively prevent the occurrence of ACI,minimize the probability of disability anddeath of patients and improve the quality of life of patients.
