Objective At present, the commonly used blood glucose detection method in clinic is the rapid blood glucose detection by the bedside of the department, also called finger-tip blood glucose detection, and the testing tool is portable blood glucose instrument.Laboratory studies have shown that hematocrit is one of the important factors affecting the accuracy of fingertip blood glucose detection, but there are great changes in perioperative hematocrit in patients with renal transplantation. Therefore, the purpose of this study was to investigate the effect of perioperative hematocrit on the accuracy of fingertip blood glucose in renal transplantation patients. Methods In this study,30 renal transplantation patients from January to December 2018 were included in the study. The methods of repeated measurement were used to collect fasting venous blood at 0,1,2,3 days after operation respectively,to detect hematocrit and biochemical blood sugar. At the same time, the blood samples of fresh venous blood and fresh fingertip capillaries were detected simultaneously in 15 seconds by three portable blood glucose instruments, which were commonly used in clinic and had different range of application forhematocrit. The results were divided into seven groups. The gold standard daily biochemical blood glucose was used as the control group while the other six groups SD-v,AC-v,GA-v,SD-cap,AC-cap and GA-cap as the observation groups. The paired t test was used to compare the two groups. Results In this study, the hematocrit of renal transplantation patients was measured 120 times,97.50% was lower than the normal range. As a whole, there was no significant difference between the SD-v group and the control group when the sample was venous blood.but there was significant difference between the other observation groups and the control group, and there was significant difference between the two groups. When the samples were fingertip capillary blood, there were statistical differences between the observation group and the control group.Differences between observation groups,SD-cap group and AC-cap group There was no significant difference between SD-cap group and GA-cap group,AC-cap group and GA-cap group. After the hematocrit was further stratified, when the hematocrit was less than 25,the difference between the SD-v group and the control group was statistically significant, but the difference between the SD-cap group and the AC-cap group was statistically significant. Conclusions Compared with the normal range, the perioperative hematocrit of renal transplantation patients is lower than that of the normal range, and even lower than the allowable range of portable blood glucose instrument commonly used in clinic.Abnormal hematocrit during perioperative period in patients with renal transplantation affects the accuracy of portable blood glucose meter. When the hematocrit is less than 25%,there is an error in the accuracy of each blood glucose meter. For the perioperative population with abnormal hematocrit, medical and nursing staff should consider the influence of hematocrit when using portable blood glucose meter to detect blood glucose,so as to improve the accuracy of blood glucose detection.
