Objective:To evaluate the application value of multidisciplinary collaborative nursing model in perioperative type 2 diabetes patients with blood glucose control level,self-management behavior,diabetes knowledge,mental state,perioperative hypoglycemic event rate,hospitalization days,and hospitalization expenses.Provide evidence for the quality of blood glucose management and the clinical promotion of multidisciplinary collaborative nursing mode in patients with type 2 diabetes during perioperative period.Methods:This study used a class of quasi-experiment research method.From April to November 2018,113 patients with type 2 diabetes who underwent elective surgery in urology,orthopedics,plastic surgery,otolaryngology head and neck surgery,and ophthalmology in a top three hospital in Shanxi Province were studied.Based on the ward filled out by the admission card at the time of admission,the patients in the eastern area were selected as the experimental group and the patients in the western area as the control group.There were 55 cases in the experimental group(12 cases of urology,12 cases of orthopedics,12 cases of plastic surgery,10 cases of otolaryngology head and neck surgery,9 cases of ophthalmology),by diabetes specialist nurses,endocrinologist,each department liaison nurse,psychological consultant,dietitian,rehabilitation therapists to form a multidisciplinary team,the implementation of perioperative multidisciplinary collaboration blood sugarmanagement mode;A total of 58 patients in the control group(12 cases of urology,12 cases of orthopedics,12 cases of plastic surgery,11 cases of otolaryngology head and neck surgery,11 cases of ophthalmology),implemented perioperative routine blood glucose management mode.Differences in fasting blood glucose,postprandial 2h blood glucose,diabetes self-management behavior,diabetes knowledge,and mental status were compared between the two groups before and after the intervention,and the difference between perioperative hypoglycemic events,hospitalization days,and hospitalization costs was compared between the two groups.Results:1.Baseline comparison: There were no statistically significant differences in the scores of the general demographic data,disease-related data,glycemic control,diabetes self-management behavior,diabetes knowledge,and mental state assessment between the two groups before intervention(P>0.05),the data between the two groups is comparable.2.Blood glucose control: The comparison within the group showed that the fasting blood glucose and postprandial 2h blood glucose of the two groups were lower than those before the intervention,and the difference was statistically significant(P<0.05).The comparison between groups showed that the postprandial 2h blood glucose level of the experimental group was lower than that of the control group(P<0.05),but the fasting blood glucose was not significantly different from the control group(P>0.05).3.Diabetes self-management behavior: The comparison within the group showed that the scores of diabetes self-management behavior and each dimension of the two groups were improved compared with those before intervention,and the difference was statistically significant(P<0.05).The comparison between groups showed that the scores of the other dimensions and the self-administration scores of the experimental group were higher than those of the control group(P<0.05),there was no statistically significant difference in the scores of taking medicine as prescribed between the two groups(P>0.05).4.Diabetes knowledge score: The comparison within the group showed that the diabetes knowledge scores of the two groups were higher than those before the intervention,and the difference was statistically significant(P<0.05).In terms of diabetes knowledge compliance rate,the experimental group patients increased after intervention compared with the intervention,the difference was statistically significant(P<0.05),but the control group had no significant difference before and after intervention(P>0.05).The comparison between the groups showed that the diabetes knowledge score and the diabetes knowledge compliance rate of the experimental group were higher than those of the control group,and the difference was statistically significant(P<0.05).5.Psychological state scores: The comparison within the group showed that the total scores of mental state and the scores of each factor in the two groups were lower than those before the intervention,and the difference was statistically significant(P<0.05).In terms of the abnormal rate of psychological state,the patients in the experimental group decreased after intervention,and the difference was statistically significant(P<0.05),but there was no significant difference between the control group before and after intervention(P>0.05).The comparison between groups showed that the scores and total scores of anxiety and depression factors in the experimental group were lower than those in the control group after intervention(P<0.05),but the anger,loneliness factor score and psychological state abnormal rate were compared with the control group,the difference was not statistically significant(P>0.05).6.The incidence of perioperative hypoglycemia,hospitalization days,and hospitalization expenses: The comparison between groups showed that the number of hospital stays in the experimental group was lower than that in the control group after intervention(P<0.05).There was no significant difference in the incidence of perioperative hypoglycemia and hospitalization costs compared with the control group(P>0.05).Conclusion:Multidisciplinary collaborative nursing mode can effectively improve postprandial 2h blood glucose in patients with type 2 diabetes,improve self-management behavior and knowledge of diabetes,relieve negative psychological stress in diabetic patients,and reduce perioperative hypoglycemic events.Hospitalization days and hospitalization expenses also have certain effects,which are worthy of clinical application.
