Objective:To explore whether visceral fat index is an independent risk factor for the occurrence of hyperuricemia in community population,and to provide a basis for the prevention and treatment of hyperuricemia.Methods:Relying on the Chinese medical association launched REACTION study,participants for starting from August 2011 to 2011 as of December,and in July2014 to December 2014 in follow-up survey of dalian city community residents,and age are not less than 40 years of age,and according to the standard screening out subjects,were included in the standard object of study of 4872 cases,the survey population according to the visceral adiposity index(VAI)value quartile levels into Q1,Q2,Q3 and Q4 four groups.The height,weight,WC,MSBP,MDBP,average heart rate,BMI and VAI were calculated.Blood tests to determine TC,TG,LDL-C,HDL-C and FPG,HbA1c and serum creatinine(Cr)and blood uric acid(UA),compare the baseline between each group of gender,age,waist circumference,BMI,blood pressure and biochemical indexes such as whether there is a difference,and the quarterback group high uric acid hematic disease rates now if there is a difference between,analysis of the correlation between visceral fat index and blood uric acid.Follow-up was conducted in the second half of 2014 to record the follow-up time,assay and measurement indicators and the baseline data 3 years ago,analyze the risk factors of hyperuricemia,and compare the progress of hyperuricemia in each group.Results:1.A total of 4,872 patients were enrolled in this study through baseline data analysis,including 364 patients with hyperuricemia,with the current incidence rate of7.5%.Q1,Q2,Q3 and Q4 group high uric acid hematic disease now rates were 4.9%,5.2%,7.2%and 12.7%respectively,c~2=70.053,P<0.05,the difference was statistically significant.2.Spearman correlation analysis displayed that SUA was positively correlated with age,average heart rate,BMI,MSBP,FPG,2hPG,TC,TG,LDL-C and VAI(r=0.195,r=0.167,r=0.273,r=0.148,r=0.032,r=0.048,r=0.068,r=0.288,r=0.059,r=0.214).P<0.05,with a negative correlation with HDL-C(r=-0.242,P<0.05).3.Binary Logistic regression analysis was performed on the enrolled baseline population,with hyperuricemia as the dependent variable,age,gender,MSBP,average heart rate,FPG,VAI,and TC as independent variables for Logistic regression analysis.The results showed that VAI was an independent risk factor for hyperuricemia after adjusting for gender,age,average heart rate and TC.The risk of hyperuricemia was 1.245 times higher for each additional standard deviation of VAI in the investigated population(OR=1.245,95%CI:1.191-1.302,P<0.001).In addition,age,average heart rate and TC were also risk factors for hyperuricemia,OR value and 95%confidence interval were 1.025(1.009-1.040),1.026(1.013-1.040)and 1.284(1.153-1.430)P<0.05,and men were more likely to develop hyperuricemia than women,respectively.4.Follow-up studies were conducted in the above population in 2014,and the measurement indexes were the same as the baseline.Cox regression was performed with no hyperuricemia in 2011 and the progression to hyperuricemia in 2014 as the endpoint events,among which 626 cases had the endpoint events mentioned above,accounting for 12.9%of the follow-up population.In groups Q1,Q2,Q3 and Q4,135cases(11.1%),115 cases(9.4%),164 cases(13.6%)and 212 cases(17.3%)occurred endpoint events,respectively.Age,gender,MSBP,average heart rate,FPG,TC and VAI after follow-up were taken as covariables and stepwise backward method was used.Results showed that age,gender,TC and VAI entered the equation,(P<0.05).After follow-up,OR values and 95%confidence intervals of VAI,age and TC were 1.077(1.057-1.097),1.013(1.003-1.023),1.108(1.024-1.199),P<0.001,and men were more likely to develop hyperuricemia than women.Conclusions:1.With the increase of visceral adiposity index,the incidence of hyperuricemia tends to increase.2.Visceral adiposity index is an independent risk factor for hyperuricemia.
