Objective: The purpose of this paper is to study the changes of hospitalization expenses of cervical cancer patients and the key factors affecting the hospitalization costs.Based on the analysis of hospitalization expenses in a city in 2007-2018,this paper summarizes the methods and measures to effectively control the hospitalization expenses of cervical cancer.In order to control the unreasonable increase of hospitalization cost of cervical cancer patients,reduce the economic burden of patients' disease,to provide data support and reference basis for the rational use of medical service resources.Methods: All data related to the cost of hospitalization for patients hospitalized for cervical cancer from 2007 to 2018 are selected.In this study,the basic situation of cervical cancer patients,the trend of hospitalization expenses and hospitalization expenses prediction,the Influencing factors of hospitalization expenses were analyzed.All the data were screened and sorted by Excel 2010,and the final valid data were imported into SPSS 24.0 for statistical analysis.Descriptive analysis was used to analyze the basic situation of patients.Trend analysis was used to analyze the trend of hospitalization expenses.Using the grey system modeling software 7.0.1(GSTA 7.0.1)to correlation analyze and forecast the total hospitalization expenses and individual expenses.Non-parametric test was used to analyze the single factor of hospitalization expenses.The recursive system model were used for the multivariate analysis of hospital expenses.Results: 1?This study included 13040 patients with cervical cancer in a city from2007 to 2018.Judging from the characteristics of social demography,The average age of the patients was 50.67±11.367,and the patients were mainly between 35 and 65 years old,accounting for 82.8% of the total patients,the proportion of patients under 45 yearsold gradually increased.94.9% of the patients were married,51.8% of patients are employees of enterprises and institutions,65.5% of patients come from municipal districts.Judging from the status of hospitalization,85.2% of the patients were treated in grade A hospital.The median number of hospital day was 14.5 days.Judging from the disease characteristics and treatment,84.4% of the patients were pathologically diagnosed as squamous cell carcinoma.73.0% of the patients had other diseases,69.1%of patients underwent surgical treatment.Judging from the medical payment,urban employees / residents of basic medical insurance,a total of 60.9%.2?From 2007 to 2018,the number of patients with cervical cancer and the total expense of hospitalization showed an increasing trend,with an average annual growth rate of 12.43%?20.29%,respectively.From 2007 to 2016,the median hospitalization cost fluctuated and increased,and the median of the hospitalization costs in 2017 and2018 decreased to 13511.56 yuan?13647.60 yuan,respectively.The median number of hospital days declined year by year,from 20 days in 2007 to 10 days in 2018.3?Judging from the overall trend of changes in individual expenses of patients with cervical cancer,the expenses of materials,diagnosis,treatment,comprehensive medical services,and drugs are all showing an upward trend,with an average annual growth rate of 16.56%?13.54%?8.38%?6.59%?1.70%,respectively.From 2007 to 2012,the drug expenses increased gradually,and the drug expenses in 2013-2018 showed a decreasing trend to varying degrees.In terms of treatment expenses and comprehensive medical service expenses,the annual growth rate of 2018 was the highest,which was 55.98%and 62.5%,respectively.From 2007 to 2015,the diagnostic expenses showed an upward trend,while the diagnostic expenses from 2016 to 2018 showed a negative trend.In terms of materials expenses,materials expenses changed greatly from 2011 to 2018,with an average annual growth rate of 23.83%.4?Based on the gray relational analysis,between 2007 and 2018,the correlation between individual costs and hospitalization costs was strong to weak,as follows: drug expense(0.9876),the treatment expense(0.9447),the diagnosis expense(0.8367),the comprehensive medical service expense(0.8307),and the materials expense(0.7965).5 ? According to the GM(1,1)model,the total hospitalization expense and individual expense of cervical cancer patients from 2019 to 2023 showed an upward trend.The average annual growth rate of each expense is as follows: materials expense(8.36%),treatment expense(7.23%),diagnosis expense(6.18%),comprehensive medical service expense(4.36%),total hospitalization expense(1.94%),and drug expense(0.25%).6?By rank sum test,the age,marital status,occupation,residence location,hospital grade,admission route,hospital days,pathological diagnosis,concomitant diseases,operation and medical payment methods all have statistical significance(P <0.05).7?According to the results of recursive system model analysis,it is concluded that the first five of the top five influencing factors are surgical operation,actual hospitalization days,associated diseases,hospital grade and patient age.Conclusion: 1?The average age of cervical cancer patients was 50.67 ±11.367,and showed a trend of younger development.Because the disease may impose a huge economic burden on patients,families and society,Women in this age group are the focus of cervical cancer prevention and control.2?From 2007 to 2018,the number of patients with cervical cancer and the total cost of hospitalization increased year by year.The forecast results of expenses from2019 to 2023 also showed that the cost of hospitalization and individual expenses showed a rising trend in varying degrees.It indicates that the burden of disease and disease economic burden of cervical cancer are more serious.3?From 2007 to 2018,the expense of drug and diagnosis showed a trend of first rising and then decreasing.The cost of treatment,materials and comprehensive medical service all showed a fluctuating upward trend.The correlation between each single item cost and hospitalization cost was in the order of strong to weak: drug expense,treatment expense,diagnostic expense,comprehensive medical service expense,material expense.It shows that the new medical reform has produced a certain effect on the change of hospitalization expenses.It is necessary to further straighten out the relations and optimize the overall structure of the price system of medical services.4.The key factors influencing the hospitalization cost of cervical cancer are:operation,actual hospitalization days,associated diseases,hospital grade,patient age.In order to alleviate the economic burden of cervical cancer patients,appropriate fee control measures should be taken in view of the above factors.
