| Background:Malignant tumor is a kind of malignant disease characterized by abnormal cell differentiation,proliferation,abnormal growth and uncontrolled growth.Malignant tumor seriously endangers people's physical and mental health in our country.Due to the long treatment cycle of malignant tumor,which is difficult to cure,comprehensive treatment must be adopted.The cost of repeated hospitalization is usually much higher than other diseases,which brings serious economic burden to individual family and society.Lung cancer is one of the most malignant diseases in the respiratory system.Worldwide,lung cancer is still the first malignant tumor in incidence rate(1 1.6%)and mortality rate(1.4%).In the past two decades,the incidence of lung cancer in China has been increasing,and the incidence age has been getting younger.The direct economic burden caused by lung cancer treatment has been increasing,and the direct and indirect medical expenses have been increasing year by year.The cost of hospitalization reflects the cost of treating diseases during hospitalization,that is,the more the cost of treating diseases,the higher the cost of hospitalization.Average such confinement is to point to in a certain period of time,the length of average hospital stay in patients during hospitalization,to a certain extent,can reflect the medical quality and medical technical level,the hospital internal integrated management and operation mechanism,is reaction hospital efficiency and effectiveness of comprehensive index,it not only reflects the length of hospital stay,also embodies the hospital the medical service ability.Shortening the average length of stay can effectively reduce the cost of hospitalization and other related costs,improve the turnover rate and utilization rate of hospital beds,increase the economic income of the hospital,and bring greater social and economic benefits to the hospital.Therefore,this study a 3 armour hospital in 2012-2018,jinan lung cancer patients in hospital costs were investigated and its influence factors were analyzed,and the evaluation of the influence factors of lung cancer,understanding the direct economic burden of lung cancer,for the future control of lung cancer patients in the hospital medical expenses growth speed,reduce the economic burden of patients with lung cancer to provide scientific basis for effectively.Methods:The data come from jinan of shandong province that a certain level of first-class comprehensive hospital health care information system,selection and partial hospital between 2012 to 2018,all of the medical record data,choose the main diagnosis by pathological biopsy standard information in hospitalized patients diagnosed with lung cancer as investigation object,and extract its electronic records in the related data.The survey included basic information about cancer patients and hospital costs.The basic information of cancer patients includes cancer type,medical record number,number of admission,gender,age,marital status,work status,medical insurance type,discharge date,length of stay,etc.Be in hospital charge includes be in hospital total cost,bed cost,nurse cost,medicine cost,operation cost,cure cost,check cost,material cost to wait.The data analysis method was used to statistically describe the basic information of the cancer patients studied,including the gender,age,marital status and work status of the patients with lung cancer.The trends of hospitalization costs and sub-costs of lung cancer patients over time were also described,and the corresponding charts were drawn.In this study,multiple linear regression analysis was used to analyze the factors that affect the cost of hospitalization to determine the impact of each factor on the cost of hospitalization and the size of the impact.SPSS 13.0 software was used for statistical processing of the data.Results:A total of 6,722 patients(27.92%)with lung cancer were selected in this study.Among them,4,667 patients were male,accounting for 69.43%,and 2,055 patients were female,accounting for 30.57%.From 2012 to 2018,the total annual hospital expenses of lung cancer patients were 21,484.13 yuan,among which the drug expenses accounted for the highest proportion(48.44%).The second is material cost,accounting for 14.99%.Nursing expenses accounted for the smallest,average about 1.84 percent.From the perspective of time trend,the proportion of bed expenses decreased gradually with time,the proportion of nursing expenses increased with time,the proportion of medicine expenses decreased with year,and the proportion of material expenses increased with year.Univariate analysis of hospitalization expenses of lung cancer patients found that marital status,medical insurance type,work status and year were statistically significant.After multiple linear regression analysis of factors influencing the hospitalization cost of patients with lung cancer,it was found that the main factors influencing the hospitalization cost of patients with lung cancer were age,marital status,type of medical insurance and discharge date.Among them,the older the age,the higher the total cost of hospitalization,and the results were statistically significant.Compared with married patients,unmarried patients had lower hospital expenses and statistical significance.In health care type,boasts town worker health care of patients with lung cancer as a reference,with the medical insurance for urban residents patients or patients at one's own expenses of hospitalization cost is relatively low and there are statistically significant,but have different health care patients in the hospital total cost is relatively high and there are statistically significant,referral of patients in the province and the town worker medical insurance patients compared with the difference was not statistically significant;The longer the length of stay in patients with lung cancer,the higher the total cost of hospitalization,and the difference was statistically significant.The hospital expense in 2012 was taken as a reference,and the total hospital expense thereafter was higher than that in 2012.Except 2014,the difference in other years was statistically significant.Conclusion:By comparing and studying the composition and changing trend of the total hospital expenses of lung cancer patients and exploring the influencing factors,it is found that the highest proportion of hospital expenses is drug expenses.The main factors that affect the cost of hospitalization are the type of medical insurance and the number of days of hospitalization.Therefore,improving the supervision mechanism of medical insurance,shortening the number of days of hospitalization and reducing the proportion of drugs are effective ways to reduce the total cost of hospitalization for cancer patients. |
