| Objective:By summarizing and analyzing the medication rule of Ye Tianshi Ointment,a representative physician in Wumen,this paper explores its influence on Reinforcing spleen and kidney herbal paste,and analyses the inheritance and innovation of the prescription rule of Reinforcing spleen and kidney herbal paste.By monitoring the systolic blood pressure(SBP)of caudal artery in elderly spontaneously hypertensive rats(SHR),examining the endothelial function,Renin angiotensin system(RAS)and observing the structural changes of thoracic aorta,we explored the hypotensive effect and possible mechanism of vascular protection of Reinforcing spleen and kidney herbal paste.Methods:1.Collect and sort out all the ointment prescriptions in Ye Tianshi's Medical Records of Clinical Guidelines,standardize the names of all the medicinal foods,use SPSS25.0 software to analyze the frequency of high-frequency medicinal foods,analyze the data according to their four scents,five flavors,efficacy and meridian tropism,summarize the law of medication,compare and analyze the similarities and differences between Reinforcing spleen and kidney herbal paste,the hypotensive ointment created by director Chen Jingwei.The similarities and differences of ointment in prescription law.Fifty 10-month-old male SHRs were randomly divided into five groups:model group(SHR-C),low-dose ointment group(SHR-L),medium-dose ointment group(SHR-M),high-dose ointment group(SHR-H),and irbesartan group(SHR-X);10 male Wistar Kyoto rats(WKY)of the same age were used as control group(WKY-C).SBP was monitored weekly during 4 weeks of drug intervention.After the experiment,the concentration of nitric oxide(NO),endothelin-1(ET-1),angiotensin ?(Ang ?),vascular endothelial growth factor(VEGF)in serum were measured.The pathological changes of thoracic aorta were observed by hematoxylin eosin staining(HE).Result:1.There are 88 kinds of traditional Chinese medicines in the ointment prescriptions in the Medical Records of Clinical Guidelines,including 80 kinds of commonly used Chinese medicines and 8 kinds of food medicines.According to the analysis of five flavors,most of them belong to the spleen and stomach meridians,heart meridians and kidney meridians.According to the efficacy analysis,the most of them are deficiency-tonifying drugs,mainly tonifying qi and yin.2.(1)During the experiment,SBP in SHR-C group was always higher than that in WKY-C group(P<0.01);SBP in ointment group and irbesartan group could be decreased after intervention.Compared with SHR-C group,the blood pressure in SHR-M group was significantly lowered in the 4th week(P<0.01),and the blood pressure in SHR-H group and SHR-X group was significantly lowered in the 1st week(P<0.05).(2)Compared with WKY-C group,the levels of serum NO and VEGF in SHR-C group decreased significantly(P<0.05),and the levels of ET-1 and Ang ? increased significantly(P<0.05);compared with SHR-C group,the levels of NO in SHR-H group increased significantly(P<0.05),Ang ? decreased significantly(P<0.05);only the levels of Ang ? in SHR-X group decreased significantly(P<0.05).(3)HE staining of thoracic aorta showed that the intima of WKY-C grorup was intact,the thickness of media was moderate,the arrangement of muscle fibers was orderly,no proliferation of vascular smooth muscle cells(VSMC)and the size of nucleus was similar;the intima of SHR-C group was incomplete,the local vascular endothelial cells(VEC)were exfoliated,the intima was obviously thicker,and the arrangement of muscle fibers was pole.The pathological changes were improved in SHR-L,SHR-M,SHR-H and SHR-X groups after intervention.The intima was relatively complete,the thickness of the medial membrane was reduced,the arrangement of muscle fibers was still disordered in varying degrees,the proliferation of VSMC was improved,but the size of the nucleus was still uneven.Morphology of thoracic aorta:Compared with WKY-C group,luminal diameter(LD),wall cross-sectional area(WCSA),luminal cross-sectional area(LCSA)and wall thickness(WT),media thickness(MT)of SHR-C group were significantly increased(P<0.001).Compared with SHR-C group,LD,WT/LD,WCSA,LCSA,WCSA/LCSA in SHR-L group decreased significantly(P<0.01),WT also decreased(P<0.05).Compared with SHR-C group,WT/LD,WCSA,WCSA/LCSA in SHR-M group decreased significantly(P<0.01),WT,LD and LCSA also decreased(P<0.05).Compared with SHR-C group,WT/LD,WCSA,LCSA,WCSA/LCSA in SHR-H group decreased significantly(P<0.01),WT,LD and MT also decreased(P<0.05).Compared with SHR-C group,WT/LD,WCSA,LCSA,WCSA/LCSA in SHR-X group decreased significantly(P<0.01),and LD also decreased(P<0.05).Conclusion:1.The medication rule of Ye Tianshi ointment can be summarized as follows:supplement is essential,supplement evil;Ganping is deficient,and the quality of Yin is thick;care for the spleen and stomach to cure many diseases;medicine and food are homologous,and complement in the middle;2.Reinforcing spleen and kidney herbal paste not only inherits the formulation of Ye Tianshi ointment,but also combines with clinical development and innovation;3.Reinforcing spleen and kidney herbal paste can effectively reduce systolic pressure,possibly by improving endothelium.Impairment of function,inhibition of over-activation of RAS system and improvement of vascular remodeling can be achieved.4.The mechanism of protective effect of Reinforcing spleen and kidney herbal paste may be achieved by increasing NO content and lowering Ang ?
