The integrated health care system,referred to as the medical alliance,is a topic of widespread concern in the medical and health industry and the society.It is also an important measure and institutional innovation for deepening medical reform in China.The medical association emphasizes the government's leadership,the sharing of resources among medical institutions within the organization,and the up-and-down linkages,aiming to promote the downward shift of medical and health care and the sinking of resources,continuously improve grassroots service capabilities,and improve the overall effectiveness of the medical and health service system.Since the implementation of the new round of medical and health system reform,China's universal medical security system has been rapidly established and continuously improved.The medical service conditions of grassroots medical and health institutions have been significantly improved,and the service capacity has gradually increased.However,the “strong grassroots” in China's medical improvement process is gradual.The process of slow development.As a model for optimal allocation of medical and health resources,medical associations are conducive to improving the construction of health service systems,accelerating the implementation of community first consultation,two-way referral and graded diagnosis and treatment,which is conducive to effectively improving the efficiency of medical and health resource allocation.The necessary medical and health expenses will play an important role in achieving scientific,healthy and sustainable development of medical and health care.At present,many developed countries in the world have established a comprehensive medical resource integration group.Although China's medical association construction achievements have made rapid progress in recent years,in many places,such as overall value orientation,medical resource allocation and service,Lack of fairness and public welfare,residents' lack of awareness of the medical association,weak sense of two-way referral between doctors and patients,and ethical issues such as inadequate compensation mechanism for medical associations.The opportunities and challenges of the construction and development of medical associations coexist,and hopes and difficulties coexist.The healthy and benign operation of the medical association needs to accumulate experience and enhance understanding in practice,and effectively embark on a path of medical and health development that suits China's national conditions.Based on the status and experience of the international medical association model construction,this paper analyzes the motivations of the medical association from the government,hospital and patient level,and reveals the ethical principles that should be followed in the development of the medical association.The principle of public welfare,the principle of people-oriented,the principle of combining fairness and efficiency,and the principle of respect for autonomy,on the basis of which the use of responsibility ethics and management ethics theory to analyze the ethical issues in the current construction and development of China's medical associations,and finally conclude the doctors Under the guidance of the ethical principles to be followed by the construction of the consortium,combined with the experience of the construction of the international medical association model,the countermeasures and suggestions for solving the ethics of the medical association in China are given: Emphasizing government responsibility and maintaining fairness and justice in the integration of medical resources;The ethical entity of each medical and health institution promotes the balance of responsibility and interests among the members;“people-oriented” to build a medical association,build a harmonious relationship between doctors and patients;clarify the individual's ethical responsibility in safeguarding the right to health,respect the patient's autonomy in medical treatment;Joint compensation incentive mechanism,etc.The aim is to establish a sustainable and integrated medical association model that is fair and reasonable,in line with the current status of China's health resources distribution,and further improve the medical and health service system by building a platform for sharing resources,benefit-winning,information exchange and responsibility sharing.Improve the service capacity of grassroots medical and health institutions,so that the medical associations can produce good social and economic benefits,thus realizing the goal of basic medical and health care for all.
