Objective: In this paper,I will discuss the distribution of HPV subtypes in cervical lesions in Da Lian,to speculate the relationship between HPV subtype and the development of cervical lesions.Moreover,analyze the postoperative pathological grading(differentiation degree)of HPV positive cervical cancer patients,to speculate the correlation between HPV and cervical cancer pathological grading.So as to provide theoretical data basis for the prevention and treatment of cervical cancer in local region.Methods: In this paper,patients with cervical lesions who visited The second hospital of Da Lian Medical University from January 2014 to June 2018 were selected as objects.There is 1094 cases has been diagnosed.Among these cases,290 cases has been diagnosed the low-grade cervical intraepithelial lesions(LSIL)by colposcopy cervical biopsy or cervical conization histopathology,448 cases has been diagnosed High-grade cervical intraepithelial lesion(HSIL),and 356 cases has been diagnosed Postoperative pathology confirmed invasive cervical cancer(ICC).Detection of HPV typing use Hybribio Company's developed kit,which use gene amplification and conduction hybridization,to find 16,18,31,33 and other 15 high-risk HPV subtypes and 6,11,42 and other 6 low-risk HPV subtypes.And analyze the distribution of HPV subtypes,multiple infection rate and pathological grade of cervical cancer.Results: 1.In all subjects(No distinction between single and mixed infections),the HPV subtypes distributed three groups as follow:1.1 There were 290 cases in the LSIL group,and the top five HPV types were HPV16 :99 cases(34.1%),HPV58:47 cases(16.2%),HPV52 : 39 cases(13.4%),HPV53 : 23 cases(7.9%),HPV18 and HPV51 each are 21 cases(7.2%).1.2 There were 448 cases in the HSIL group,and the top five were HPV16: 277 cases(61.8%),HPV58: 60 cases(13.4%),HPV33: 49 cases(10.9%),HPV52: 40 cases(8.9%),and HPV18: 25 cases(5.9%).1.3 The ICC group had 356 cases,and the top five were HPV16: 249 cases(69.9%),HPV18: 34 cases(9.6%),HPV58: 22 cases(6.2%),and HPV33: 18 cases(5.1%).1.4 HPV negative : 4.8% in the LSIL group,4.5% in the HSIL group,9.8% in the ICC group2.In the single-infected each team,the top four proportion of HPV subtypes infection in the ICC group distributed as follow:2.1 HPV16: there are 64 cases in LSIL group.(Proportion is 33.3%),189 cases in HSIL group.(Proportion is 60%),204 cases in ICC group.(Proportion is 69.6%).The proportion is increased by the grade of cervical lesions.Moreover,the difference was statistically significant(P<0.05).2.2 HPV18: there are 8 cases in LSIL group.(Proportion is 4.2%),12 cases in HSIL group.(Proportion is 3.8%),25 cases in ICC group.(Proportion is 8.5%).In LSIL and HSIL groups,the proportion is low,but in ICC group,the proportion is high.Moreover,there are statistical differences between the two comparisons(P<0.05).2.3 HPV33: there are 8 cases in LSIL group.(Proportion is 4.2%),26 cases in HSIL group.(Proportion is 8.3%),11 cases in ICC group.(Proportion is 3.8%).Moreover,the difference was statistically significant(P<0.05)in each group.The highest proportion is in HSIL group.2.4 HPV58: there are 33 cases in LSIL group.(Proportion is 17.2%),25 cases in HSIL group.(Proportion is 7.9%),10 cases in ICC group.(Proportion is 3.4%).Moreover,the difference was statistically significant(P<0.05)in each group.The highest proportion is in LSIL group.2.5 In HSIL and ICC group,there was not occurred by single ingle infection of low-risk HPV.2.6 In each group of cervical lesions.Negative HPV accounted for a certain proportion,which is 11.9%.Moreover,the difference was statistically significant(P=0.037,<0.05).3.Multiple infections were significantly different in different pathological grades,with statistical significance(P<0.05).It can be seen from the data of multiple infection with HPV16 type,that with the increase of cervical lesion grade,the multiple infection rate gradually decreases,and the cervical cancer group has the lowest,and the difference between the three groups is statistical significant(P<0.05).4.Distribution of HPV16 single infection in different pathological grades(differentiation degree)in ICC group: There were 6 cases of high differentiation,64 cases of medium differentiation and 65 cases of low differentiation,and the differences between the groups were not statistically significant(P>0.05).Among the other two cervical cancer groups,the most common single infection subtypes were HPV33 and 58,and the differences between the two groups were not statistically significant(P>0.05).Conclusion: 1.There are significant differences in the distribution of HPV subtypes at different grades of cervical lesions.The most common HPV subtypes in LSIL patients were 16,58,52 and 53.The most common HPV subtypes in HSIL patients were 16,58,33 and 52.The most common HPV subtypes in ICC patients were 16,18,58 and 33.The proportion of HPV negative was higher in ICC group.Negative HPV accounted for a certain proportion in each group(LSIL group is 4.8%,HSIL group is 4.5% and ICC group is 9.8%).2.The infection rate and pathogenicity of different HPV subtypes are very different: HPV 16 has the highest infection rate and strongest carcinogenicity.HPV18 has strong carcinogenicity.HPV33 and 58 have certain carcinogenicity and the low-risk HPV has weak carcinogenicity.3.HPV multiple infection,with the increase of cervical lesion grade,the proportion gradually declined,and single infection is more carcinogenic than multiple infection.4.There was no correlation between HPV and pathological grade of cervical cancer.
