Objective:To determine the value of endobronchial ultrasound in the diagnosis of lung cancer and whether it has an effect on the prognosis of patients with lung cancer.Methods:from 2013.01 to 2015.12,all patients with lung cancer diagnosed pathologically by endobronchial ultrasound in the Respiratory Department of Hospital were collected.And non-ultrasonic bronchoscopic diagnosis,age,sex matching,lung cancer clinical stage?to?patients,collect detailed and complete clinical data,diagnosis and treatment process,using retrospective method for statistical analysis.The differences of clinical stage,TNM stage,selected treatment methods and survival time between the two groups were statistically analyzed to determine whether has an effect on the prognosis of patients.At the same time,Logistic regression model was used to analyze the related factors of lung cancer patients.Results:1.In the endobronchial ultrasound group,there were 162 patients,including 54cases(33.3%)of adenocarcinoma,34 cases(21.0%)of squamous cell carcinoma,68cases(42.0%)of small cell carcinoma,and 6 cases(3.7%)of other types,including 2cases of adenosquamous carcinoma.There were 3 cases of mixed carcinoma and 1 case of undefined pathology.In the non-ultrasonic bronchoscopy group,there were 107cases of adenocarcinoma(73.8%),21 cases of squamous cell carcinoma(19.6%),1case of small cell carcinoma(0.9%)and 6 cases of other types(5.7%),including 4cases of adenosquamous carcinoma.There were 2 cases of mixed carcinoma.In EBUS group,19 cases were examined by EBUS before surgical treatment,including 10 cases of adenocarcinoma(52.6%),8 cases of squamous cell carcinoma(42.1%)and 1 case of small cell carcinoma(5.3%).2.The results of this study showed that 162 patients in the ultrasonic bronchoscopy group,combined with other imaging examinations,There were 5 cases of T1N1M0,5 cases of T2N1M0,1 case of T3N0M0,5 cases of T1N2M0,6 cases of T2N2M0,1 case of T3N1M0,4 cases of T3N2M0,2 cases of T4N1M0,1 case of T1N3M0,7 cases of T2N3M0,2 cases of T3N3M0,5 cases of T4N2M0 and 7 cases of T4N3M0.There were 64 cases of T1-4N0-3M1 and 48 cases of undefined TNM staging.There were 11 cases of stage?,13 cases of stage?,41 cases of stage?,72cases of stage?and 25 cases of non-staged patients.There were 51 cases of T1N0M0,25 cases of T2N0M0,10 cases of T1N1M0,9 cases of T2N1M0,1 case of T2N0M1,2cases of T1N2M0,1 case of T2N2M0,1 case of T3N1M0,1 case of T3N2M0,3 cases of T3N3M0,1 case of T4N3M0 and 1 case of T4N2M0.Clinical stage:50 cases of stage?,There were 46 cases in stage?,11 cases in stage?and 0 cases in stage?.In the ultrasonic bronchoscopy group,there were 19 cases of ultrasonic bronchoscopy before surgical treatment.in TNM stage,there were 6 cases of T1N0M0,4 cases of T1N1M0,1 case of T1N2M0,6 cases of T2N0M0 and 2 cases of T2N1M0.Stage?7 cases,There were 2 cases in stage?and 0 cases in stage?.3.The 1,3 and 5 year survival rates were 76.9%,32.3%and 12.2%,respectively.the average survival time was(28.427±2.487)months.The 1,3 and 5 year survival rates of non-ultrasonic bronchoscopy patients were 100.0%,58.9%and 36.0%,respectively,the average survival time was(45.888±1.736)months.there was significant difference between the two groups(x~2=24.692 P=0.006).19 cases in the ultrasonic bronchoscopy group were treated surgically.the 1,3 and 5year survival rates were 100%,89.5%and 28.3%respectively,and the average survival time was(51.217±2.833).Compared with the non-ultrasonic bronchoscopy group,the1 year,3 year and 5 year survival rates were 100%,89.5%and 28.3%,respectively.There was no significant difference in survival time(x~2=1.494,P=0.222).4.In the survival analysis of 269 patients with lung cancer,sex(P=0.009),pathological type(P=0.002),T stage(P=0.000),N stage(P=0.000)and treatment(P=0.000)all affected the survival of patients with lung cancer.The treatment(P=0.000)was an independent risk factor for the survival of patients with lung cancer.Conclusion:The survival rate of patients undergoing ultrasonic bronchoscopy was lower than that of patients undergoing non-ultrasonic bronchoscopy,which was related to the clinical stage of EBUS group was not suitable for surgical treatment.The survival of patients with T1N1M0 non-small cell lung cancer undergoing preoperative endobronchial ultrasound was higher than that of patients with non-endobronchial ultrasound.Sex,pathological type,T stage,N stage and treatment can affect the survival of lung cancer,in which treatment is an independent risk factor for the survival of patients with lung cancer.
