Objective: Clinical comparison of thoracoscopic and open lobectomy in patients with stage ? to ? lung cancer.Method: To explore 43 patients underwent thoracotomy and 142 patients underwent thoracoscopic surgery.Compare with the differences in postoperative clinical indicators between the two groups.Contrast with the operative conditions(duration of operation,amount of intraoperative blood loss,number of lymph node dissection),postoperative conditions(pathological results,drainage volume in the first three days,duration of catheterization)and 2-year postoperative survival rate and metastasis rate between the two groups.Results:1.There were no statistically significant differences between the two groups in the general data.2.There were statistically significant differences between the two groups in lesion diameter,number of lymph node dissection,pathological classification and postoperative pathological stage(P<0.05).3.The two groups in lesion diameter,number of lymph node dissection,pathological classification and postoperative pathological stage were statistically significant differences(P<0.05).4.Thoracoscope surgery in patients with?a-?a period more than the number of patients with open thoracic surgery.In phase ?b-?b is less than the number of patients with open thoracic surgery.The difference between the two groups was statistically significant(P<0.05).5.The 2-year survival rate(90.8%)in the thoracoscopic group was significantly higher than that in the thoracotomy group(76.7%),and the difference was statistically significant(P<0.05).The 2-year recurrence rate(13.4%)was significantly lower than that in the thoracotomy group(30.2%),and the difference was statistically significant(P<0.05).Conclusion:Compared with open lobectomy,thoracoscopy has the characteristics of shorter operation time,less surgical trauma and faster postoperative recovery,which is in line with the current concept of rapid rehabilitation surgery.Lobectomy is more advantageous for lung cancer patients with larger tumor and later stage,which is worthy of clinical application.The weakness of this study is that the follow-up time is relatively short and the number of patients undergoing thoracotomy is relatively small,which further confirms the need for a multi-center randomized controlled study with a large sample size.Further studies are needed to improve the long-term prognosis of patients by choosing surgical methods.
