Objective:So far there is a clear consensus on the healing process after tooth extraction, extraction socket filled with blood clots are gradually replaced by new bone. This biological cascade is accompanied by a decrease in height and width of the alveolar bone, resulting limited bone volume and thus affecting the subsequent treatment of dental implants. For this reason, many studies have investigated the placement of different bone graft materials in the tooth extraction socket, such as autograft, xenograft or allograft, and have been shown to preserve alveolar bone height and width.Up to present, the composite of collagen and calcium phosphate is applied to the sites, after tooth extraction,to guide the regeneration of bone and gingiva. The clinical effects are still controversial and no consensus has been reached. The purpose of this study was to investigate the clinical effects of collagen calcium phosphate composite on socket preservation by observing and analyzing clinical cases. Methods:A total of 20 cases were collected in this study. The data collection time was from October 2016 to October 2017 and all the patients were diagnosed as alveolar bone defects after tooth extraction. The alveolar bone preservation were conducted by using socket preservation technique. In the test group,10 sockets were filled with Horien Collagen Bone Graft Matrix alone and Horien Collagen Bone Graft Matrix coveredwith SIS membrane were used in other 9 sockets. The control group included 10 cases11 sockets without inserting any materials after tooth extraction.Follow-up visits were conducted at 1 month,6 months, and 12 months after socket preservation treatment.Questionnaires, clinical observations, and CT(computer tomography)measurements were used to investigate alveolar bone alterations, gingival healing process, pain score, patient satisfaction index and complication. Results:According to the statistics of the results of alveolar bone preservation, gingival healing process, pain score, patient satisfaction and complications on 20 cases after socket preservation surgery,it was found that the use of collagen calcium phosphate composite(Horien Collagen Bone Graft Matrix)resulted in good wound healing and bone regeneration in the treatment of alveolar bone defects and soft tissue atrophy. Two group results showed that the use of collagen-calcium phosphate composites alone and without, the mean value of buccal bone wall height was 1.00 mm and-1.79 mm,respectively, and the mean values were 0.94 mm and-1.15 mm,respectively. The mean value of buccal bone wall width was 0.36 mm and-0.37 mm and the mean values were 0.33 mm and-0.61 mm,respectively. The buccal and lingual bone wall height, and width U value is 22.5 and P value is 0.12114(P< 0.05)showed there were statistically significant difference. The results of the two groups showed that compared with those of patients who did and did not use SIS membrane to cover collagen calcium phosphate composite, the mean value of buccal bone wall height was 1.00 mm and 1.18 mm and lingual bone wall height was 0.94 mm and 0.05 mm,respectively. The mean value of buccal bone wall width was 0.36 mm and 0.26 mm,and mean value of lingual bone wall width was 0.33 mm and 0.83 mm,respectively. The buccal and lingual bone wall height, and the buccal and lingual bone wall width U value is 9 and P value is 0.5287(P>0.05)showed no statistically significant difference. After CT and statistical analysis of the results showed patients treated with collagen calcium phosphate composite had significant preserved alveolar bone volume. It was also observed that the height and width of alveolar bone were significantly reduced in patients who did not use bone graft material.Patients that were treated with collagen calcium phosphate composites had better soft tissue healing, the least pain and highest satisfaction. In terms of overall treatment effect, the use of collagen calcium phosphate composites also shows noticeable advantages in terms of simplicity, comfort, and prognosis of patients. Conclusion:Collagen calcium phosphate composite(Horien Collagen Bone Graft Matrix)has shown an noticeable effect on the repairing of alveolar bone defects and soft tissue recession,it should be implemented clinically according to the needs of the patients,as an alternative good solution to treat alveolar bone defects and tissue recession. The Horien Collagen Bone Graft Matrix combined with the SIS membrane is an optional surgical method for soft and hard tissues regeneration.
